对侧刺激声影响畸变产物耳声发射临床意义的探讨

摘  要:目的:观察白噪声对畸变产物耳声发射输入输出函数曲线(I/O)影响,探讨其临床应用价值。方法:检测临床患者61例,包括单纯性耳鸣不伴有听力下降患者32例;以低频听力下降为主的耳聋患者27例;蜗后病变2例。结果:耳鸣患者中21例、低频感音神经性聋中19例及蜗后病变2例的对侧抑制现象消失。结论:对侧抑制现象对于耳鸣的病因学探讨、低频感音神经性聋的定位及蜗性蜗后性病变的鉴别上都具有重要的临床应用价值。同时也可看出I/O曲线在临床应用上更直观、准确。Objective:To observe the change of Iuput Output of DPOAE with contralateral acoustic stimulation of white noise (WN) and study its clinical significance.Methods:61 patients were studied,induding:(1)32 patients with tinnitus but normal hearing,(2)27 patients with low frequency sensorineural hearing loss,and (3) 2 patients with retrocochlear deafness.Results:I/O of DPOAE suppressed phenomenon disappeared in 21 patients with tinnitus,19 patients with low frequency sensorineural hearing loss and 2 patients with retrocochlear deafness.Conclusion:I/O of DPOAE is valuable in studying mechanism of tinnitus,location of low frequency sensorineural hearing loss and distinguishing cochlear hearing loss and retrocochlear hearing loss,and it offers a more precise and direct perception in clinical application.
