强直性脊柱炎患者外周血T细胞凋亡及亚群极化分析

摘　　要：目的探讨外周血T细胞凋亡以及T细胞亚群极化在强直性脊柱炎患者免疫发病机制中的作用。方法CT检查患者骶髂关节并分为Ⅰ、Ⅱ和Ⅲ级。尼龙棉柱法分离患者外周血T细胞,PI染色结合流式细胞术检测T细胞凋亡率,流式细胞术测定CD4+CD30+T、CD4+CD30-T、CD8+CD30+T、CD8+CD30-T细胞百分率。结果与健康对照者相比,AS患者外周血T细胞凋亡率明显降低(P<0.05)。Ⅰ和Ⅱ级患者外周血CD4+T细胞和CD4+T/CD8+T比值均明显降低(P<0.05),CD8+T细胞百分率明显升高(P<0.05)。AS患者外周血CD4+CD30+T无明显变化;而CD8+CD30+T均明显高于健康对照者(P<0.05)。Ⅰ、Ⅱ级患者CD4+CD30-/CD4+CD30+比值明显低于健康对照者(P<0.05);Ⅰ、Ⅱ和Ⅲ级患者CD8+CD30-/CD8+CD30+比值明显低于健康对照者(P<0.05)。结论AS患者外周血T细胞凋亡不足和T细胞亚群极化失衡与AS患者免疫学发病机制有关。Objective To investigate the role of apoptosis and polarization of peripheral blood T cells in ankylosing spondylitis. Methods The ankylosing spondylitis patients were diagnosed in accordance with 1984 diagnostic criteria of ankylosing spondylitis and divided into grade Ⅰ , Ⅱ and Ⅲ as computerized tomography（CT）. The apoptotic ratios of T cells were evaluated by FCM. T cell subsets were analyzed by immunofluorescence staining and bicolor flow cytometry by CD4, CD8 and CD30 gating. Results The apoptotic ratios of peripheral blood T cells in AS were clearly lower than healthy control （P〈0.05）. CD4＋T cells and the ratios of CD4＋T/CD8CF in grade Ⅰ and gradeⅡ were obviously lower than healthy control （P〈0.05） while CD8＋T cells were obviously higher than healthy control （P〈0.05）. CD4＋CD30＋T cells in AS were no statistical significance with healthy control while CD8＋CD30＋T cells were obviously higher than healthy control （P〈0.05）. The ratios of CD4＋CD30-T/CD4＋CD30＋T in grade Ⅰ and Ⅱwere obviously lower than healthy control （P〈0.05）; the ratios of CD8＋ CD30＋T/CD8＋CD30＋T in all AS patients were obviously lower than healthy control （P〈0.05）. Conclusion Low apoptosis and disordered polarization of peripheral blood T cells in ankylosing spondylitis might involve in the immunopathogenesis of ankylosing spondylitis .
