口服硝酸甘油片治疗冠心病临床疗效分析

摘　　要：用口服硝酸甘油方法治疗８２例冠心病，经临床观察，结果：４６例用消心痛３０ｍｇ／ｄ及３６例用５—单硝酸异山梨酯２０～６０ｍｇ／ｄ治疗，心绞痛缓介率分别为７８．５７％、８１．８２％。对缺血性ＳＴ—Ｔ改善者分别为７８．２６％、８０．５６％，两组疗效无显著差异，Ｐ＞０．０５。治疗两周后，两组药物均产生耐药性，改用口服硝酸甘油片３～６ｍｇ／ｄ经临床观察三周，心绞痛缓介有效率为９８％；缺血性ＳＴ—Ｔ改善者有效率为９７．５６％；心功能改善者７２．７３％。经统计学处理：用口服硝酸甘油比应用消心痛及５—单硝酸异山梨酯治疗有显著差异。Ｘ２分别为８．２７４、１３．３９４、Ｐ＜０．０１，它观察４～６周，心绞痛及ＳＴ—Ｔ未再出现反复现象。本组资料表明：口服硝酸甘油片对缓介心绞痛，改善心肌供血及纠正心力衰竭等方面的疗效明显的优于二硝酸异山梨醇酯（消心痛）及５—单硝酸异山梨酯（丽珠欣乐）。对该药传统的用药方法。We　have　treated　82　cases　of　coronary　heart　disease　by　giving　orally　taken　nitroglycerin　in　cohich,46　cases　were　treated　by　giving　carvasin　30mg/d　and　the　other　36　cases,by　5-isosorbidde　mononitrate　20～60mg/d.　The　clinical　observation　indicates　that　the　relieving　rates　of　angina　pectoris　are　respectively　78.57%　and　8182%.The　improving　rates　as　regards　to　blood-shortage　ST-T　are　respectively　78.26%　and　80.56%,P>0.05.The　two　groups　didn’t show　apparently　difference　in　curative　effects.Two　weeks　after　treatment　drugresistance　of both　groups　developed.Sonitroglycerin　pills　3～6mg/d　cases　given.Afrer　another　3　weeks　observation,the　effective　relievin-g rate　of　angina　pectoris　was　98%,the　effective　rate　of　the　improved　cases　of　bloodshortage　STT　was　97.56%　and　of　cardiofunction　72.73%.Statically　the　application　of　orally-taken　nitrolycerin　is　noticeably　different　from　that　of　carvasin　and　Sisoiorbide　mononitrate.X2　are　respectively　8.272　and　13.394,P<0.01.In　the　following　4～6　weeks,no　relapse　of　anfina　pectoris　and　ST-T　cuas　observed.It　is　thus　indicated　that,in　relieving　angina　pectoris,improving　heart　muscles　blood　supply　and　rectifing　heart　failure,nitroglycerin　pills　are　remarkably　superior　to　carvasin　and　S-isosorbide　momomitrate.Further　research　is　respected　for　the　traditional　application　of　this　medicine.
