摘 要:目的探讨免疫表型检测对骨髓增生异常综合征(MDS)诊断及鉴别诊断的价值。方法采用免疫细胞化学法中的生物素-亲和素桥联碱性磷酸酶酶标法(ABC-AP法)对38例初诊的MDS患者、8例再生障碍性贫血(AA)患者及17例正常对照的骨髓片进行免疫表型检测。结果CD41阳性的淋巴样小巨核细胞在MDS组中较特异出现,且以RCMD组出现率最高;MDS患者骨髓单个核细胞(MNC)早期髓系抗原CD13、CD33表达升高,而T淋巴系抗原CD3表达下降,其主要表现在RAEB和RCMD组,而RA/RAS组不明显;干/祖细胞抗原CD34在MDS组未见明显增加。结论酶标淋巴样小巨核及其他免疫表型检测有助于MDS的诊断及鉴别诊断。Objective To evaluate the value of immunophenotyping in the diagnosis and classification of myelodysplastic syndrome(MDS). Methods The immunophenotyping of 38 preliminary diagnosis MDS patients,8 aplastic anemia patients and 17 normal persons were detected by one of the immunocytochemical methods which is biotin-avidin linked alkaline phosphatase enzyme linked immunosorbent assay. Results Lymphoid micromegakaryocytes with CD41 + specially appeared in MDS group and showed the highest frequency in RCMD group and showed the highest frequency in RCMD group. CDI3 and CD33 which were mononuclearcell nonage medullary system antigen expression increased in MDS patients while inversely T systema lymphaticum antigen CD3 expression decreased. This condition mainly appeared in RAEB and RCMD groups and was not obviously in RA and RAS groups. Stem cell/progenitor cell antigen CD34 expression did not increased in MDS group. Conclusion Detecting lymphoid micromegakaryocyte and other immunophenotying is helpful for early diagnosis and classification.
