早期糖尿病视网膜病变进展的危险因素

摘　　要：目的探讨早期糖尿病视网膜病变(DR)进展的相关危险因素。方法收集DR-0期,非增生性糖尿病视网膜病变(NPDR)轻、中、重期各30例,正常对照组40例,记录年龄、病程、血糖、血压、血脂、血小板参数、凝血功能在不同分组中的测量值,分析其与早期DR发生发展的关系。结果DR组的糖化血红蛋白(HbA1C)、空腹血糖(FBG)、收缩压(SBP)、胆固醇(CHO)、甘油三酯(TG)、低密度脂蛋白(LDL)及纤维蛋白原(FG)明显高于正常对照组,而高密度脂蛋白(HDL)则低于对照组;且病程、HbA1C、TG与DR的病变程度呈显著正相关。结论HbA1C、FBG、SBP、CHO、TG、LDL、FG是DR的危险因素,且病程、HbA1C、TG可能与DR的发展有相关性。Objective Diabetic retinopathy （DR） is one of the most frequently blindness-leading eye ground diseases ,but its cause is not clear yet. It is very important to study the influences and functions of related systemic factors. The aim of this study is to investigate the related risk factors of early DR. Methods In this study the early DR is divided into four periods which include DR-0 period,mild,moderate and severe degree of Non-proliferative diabetic retinopathy （NPDR）.An examination outcomes from 30 cases of each DR group and 40 cases of normal control group including age,course of disease,level of blood glucose,systolic blood pressure,diastolic blood pressure,serum lipid level,platelet parameter and blood coagulation function were recorded. Analysis of its relationship with the progression of early diabetic retinopathy. Results HbA1C and FBG of DR Group was significantly higher than the normal control group（P 0.01）,SBP、CHO、TG、LDL and FG of DR Group was enhenced in comparison with the normal control group（P 0.05）;while HDL is lower than the control group（P0.05）. And the course of DR,HbA1C、 TG and the process degree of DR demonstrate a significant positive correlation（P0.01）. Conclusion HbA1C、FBG、SBP、 CHO、TG、LDL and FG are major risk factors for the occurence of DR,the course、HbA1C 、TG,may be related to the development of DR. It is extremely important to control the related risk factors in the occurrence and development of DR.
