摘 要:目的探索化疗致粒细胞缺乏症者感染发生的危险因素,为临床治疗提供科学的参考依据。方法应用病例对照研究,256例因化疗致粒细胞缺乏症的恶性肿瘤患者中发生感染的42例患者为病例组,未发生感染的214例患者为对照组;对有关感染的影响因素进行单因素2χ检验和多因素非条件Logistic回归分析。结果多因素分析显示化疗后早期低淋巴细胞计数、中性粒细胞最低值<0.2×109/L和化疗第1周期差异有统计学意义,OR值分别为3.347、2.537、2.135。结论化疗后早期低淋巴细胞计数、中性粒细胞最低值<0.2×109/L和第1周期化疗是化疗致粒细胞缺乏症患者感染的危险因素。OBJECTIVE To explore the risk factors of infection in patients with chemotherapy-induced neutropenia in order to offer scientific references for clinical treatment. METHODS With ease-control study, two hundred and fifty-six cancer patients with chemotherapy-induced neutropenia were compared between the infection group and non-infection group. The data were analyzed on risk factors by univariate analysis (Chi-square test) and multivariate analysis (non-conditional Logistic regression models). RESULTS Multivariate analysis showed that early lymphopenia after chemotherapy (OR=3. 347), the depth of the absolute neutrophil count nadir(〈0. 2 × 10^9/L) (OR= 2. 537) and the first cycle of chemotherapy (OR = 2. 135) were the significant risk factors. CONCLUSIONS Early Iymphopenia after chemotherapy, the depth of the absolute neutrophil count nadir(〈0.2 × 10^9/L) and the first cycle of chemotherapy are risk factors of infection with chemotherapy-induced neutropenia.
