摘 要:目的研究类风湿关节炎(RA)患者血清抗环瓜氨酸肽(CCP)抗体水平与骨关节侵蚀的相关性。方法检测75例RA患者与25例非RA患者血清中抗CCP抗体与IgM类风湿因子(RF)水平,并用放射学方法检测其关节侵蚀情况,分析结果的相关性。结果抗CCP抗体诊断RA的敏感性为64.0%,特异性为92.0%,比数积为20.4。骨关节侵蚀诊断RA的敏感性为54.7%,特异性为76.0%,比数积为3.8。RF诊断RA的敏感性为61.3%,特异性为76.0%,比数积为5.0。抗CCP抗体与RF和骨关节侵蚀有良好的相关性。结论抗CCP抗体是RA诊断的良好指标,对患者骨关节侵蚀的预测可能有重要意义。Objective To explore the relationship between anti-cyclic citrullinated peptide antibody (anti-CCP) and articular erosion in patients with rheumatoid arthritis (RA). Methods The rate-nephelometry was used to detect rheumatoid factor, ELISA was used to measure the level of anti-CCP antibody, and X-ray photograph was used to check articular erosion. Results The diagnostic sensitivities of anti-CCP antibody, articular erosion and rheumatoid factor were 64.0%, 54.7% and 61.3% respectively, the diagnostic specificities of anti-CCP antibody, articular erosion and rheumatoid factor were 92.0%, 76.0% and 76.0% respectively, and odd product were 20.4, 3.8, 5.0 respectively. There was strong relativities between anti-CCP antibody in RA and articular erosion. Conclusions Anti-CCP antibody was a satisfactory marker for RA, and it may have an important role to predict the articular erosion in RA.
