摘　　要：目的:探讨ω3多不饱和脂肪酸对脓毒症大鼠骨骼肌高分解代谢的影响。方法:30只SD大鼠随机分组,模型组采用盲肠结扎穿刺术复制脓毒症模型,假手术组仅翻动盲肠,不做结扎处理。试验期间大鼠禁食不禁水,试验组给予加用ω3脂肪酸的肠外制剂,药物对照组除了ω3脂肪酸外,其余成分相同。其余仅给予生理盐水肠外注射。在模型复制前5d即开始连续给相应治疗。造模第16小时,无菌操作下取双侧伸趾长肌,一侧放入液氮中保存,另一侧放入孵育液中继续孵育2h,并测定湿重。高效液相色谱法测定肌肉组织中3甲基组氨酸(3-MH)含量。结果:各组一侧伸趾长肌湿重差异未见显著性(P>0.05)。与正常组比较,脓毒症组16h点伸趾长肌降解率升高明显(P<0.05)。与脓毒症组比较,ω3脂肪酸组大鼠3-MH含量均下降明显(P<0.05)。结论:脓毒症时存在骨骼肌高分解代谢,ω3脂肪酸可明显降低骨骼肌高分解代谢率。Objective To explore the effect of ω3 polyunsaturated fatty acids on hypercatabolism in the skeletal muscle in septic rats.Methods 30 SD mice were randomized to sham operation group,sepsis group,treatment group,total paraenteral nutrition（TPN） group,or blank control group.The treatment group were administered parenteral nutrition plus ω3 fatty acid,while the TPN group received paraenteral nutrition alone.Other groups only received parenteral normal saline.Corresponding treatments initiated 5 days before model establishment.Bilateral extensor digitorumlongus were resected 16 h following model establishment.One was stored in liquid nitrogen;the other was incubated for 2 hours in the incubating solutions and the wet weight was measured.3-MH contents of the extensor digitorumlongus were detected by high-performance liquid chromatography（HPLC）.Results The wet weight of the extensor digitorumlongus did not differ significantly among the groups（P〉 0.05）.As compared with the blank control group,the degradation rate of the muscle at 16 h increased markedly in the sepsis group（P〈 0.05）.3-MH content in the treatment group was declined sharply,as compared with the sepsis group（P〉 0.05）.Conclusion Hypercatabolism is present in sepsis and ω3 fatty acid may reduce the rate of hypercatabolism notably.
