手术治疗胫骨平台骨折48例临床分析

摘　　要：目的探讨手术治疗胫骨平台骨折的临床疗效。方法回顾性总结本院1998年1月—2008年1月收治的胫骨平台骨折48例的治疗效果。按Schatzker分类,Ⅰ型8例,Ⅱ型30例,Ⅲ型5例,Ⅳ型3例,Ⅴ型1例,Ⅵ型1例,均采用切开复位内固定治疗。结果42例获得随访,随访期1-5年,全部骨性愈合。结论手术是治疗胫骨平台骨折的有效方法,解剖复位、骨缺损植骨、稳定的固定和早期功能锻炼是减少并发症的关键。Objective To investigate the clinically therapeutic results of internal fixation in treatment of tibial plateau fractures.Methods Forty-eight cases of tibial plateau fractures were treated from January 1998 to January 2008 in our hospital.According to Schatzker classification,the cases were diagnosed as type Ⅰ（n=8）,type Ⅱ（n=30）,type Ⅲ（n=5）,type Ⅳ（n=3）,type Ⅴ（n=1） and type Ⅵ（n=1）,respectively.The patients were treated with open reduction and internal fixation.Results Forty-two cases were followed up from 1 to 5 years.All the fractures healed up unitedly.Conclusion Internal fixation is an effective method to treat tibial plateau fracture,and the operation procedure should vary with fracture types.Postoperative complications are closely associated with fracture complexity.Anatomical reduction,bone transplantation,stable internal fixation and early functional exercises are effective measures to preventing them.Early treatment of soft tissue injury must be emphasized especially.
