老年转子间骨折手术方式的选择及其疗效分析

摘　　要：目的比较四种内固定方法治疗老年转子间骨折的疗效和并发症的发生率。方法将108例(109髋)老年转子间骨折分为四组,分别采用DHS、PFN、股骨近端解剖钢板、髋关节置换进行治疗,随访对照,评估临床疗效。结果四组中DHS组、PFN组、髋关节置换组间差异无统计学意义(P>0.05)、优于解剖钢板组(P<0.05)。结论老年转子间骨折的手术治疗,应根据病情分别选择DHS、PFN、股骨近端解剖钢板、髋关节置换,能够提高临床治愈率,降低并发症。Objective To compare the curative effect and complication rate of four different internal fixation for the treatment of aged femoral intertrochanteric fractures.Methods 108 cases of. 109 aged femoral intertrochanteric fractures were divided in four groups, respectively, using DHS, PFN, proximal femoral anatomical plate, and hip replacement treatment, and conducted follow-up evaluation of their surgical treatment, and analyzed statistically.Result In efficacy, There was no significant difference in DHS group, PFN group, and hip replacement group （P〉 0.05）, superior anatomical plate group （P〈0.05） .Conclusion According to pathogenetic condition, choose DHS, PFN, proximal femoral anatomical plate, and hip replacement in curing aged femoral Intertrochanteric fractures. It can increase recovery rate, and decrease complication rate.
