纤溶酶与小剂量尿激酶治疗下肢深静脉血栓形成的疗效和安全性比较

摘　　要：目的比较纤溶酶和小剂量尿激酶治疗下肢深静脉血栓形成的临床疗效和安全性。方法回顾分析104例下肢深静脉血栓形成治疗的临床资料,分为纤溶酶组(56例)和小剂量尿激酶组(48例),溶栓分别采用纤溶酶和小剂量尿激酶,两组其余治疗相同,包括抗凝(低分子肝素钙)、低分子右旋糖酐注射液加丹参静滴,疗程14天,比较两组下肢深静脉再通情况和疗效;观察严重出血并发症发生率,包括胃肠出血(黑便)、泌尿系出血(血尿)、颅内出血等。疗程第5d和第10d监测凝血酶原时间。结果两组再通率及临床有效率无显著差异(P>0.05),两组出血并发症及PT过度延长比较有显著差异(P<0.05)。结论纤溶酶和小剂量尿激酶溶栓治疗的效果均好,但纤溶酶使用简便、安全性可靠,具有更广阔的应用前景。Objective To compare the therapeutic effect and safety of deep venous thrombosis of lower extremities treated with fibrinolysin and small dosage urokinas. Methods Clinical data of 104 patients with deep venous thrombosis of lower extremities were retrospectively analyzed. The patients were divided into fibrinolysin group （56 cases） and small dosage urokinas group （48 cases）. The two groups were treated with fibrinolysis and small dosage urokinas, respectively. Other treatments of two groups, including anticoagulation, dextran-40 and salvia miltiorrhiza, were similar. Recanalization and therapeutic effect of deep venous thrombosis were compared. The serious hemorrhage complications including gastrointestinal hemorrhage （dark stools）, uropoiesis hemorrhage （bloody urine） and intracranial hemorrhage were ob- served. Prothrombin time was monitored on 5 days and 10 days after therapy. Result Recanalization rate and clinical effective rate of two groups demonstrated no significant differences. The hemorrhage complications and extended PT of fibrinolysin group were lower than that of the small dosage urokinas group with significant differences （P〈0. 05）. Conclusion The therapeutic effect of fibrinolysin and small dosage urokinas was good for deep venous thrombosis of lower extremities.
