摘 要:目的:探讨叶酸、维生素B_(12)对老年高血压伴高同型半胱氨酸(Hcy)血症患者的Hcy、一氧化氮(NO)、血压(BP)水平及脉搏波传导速度(PWV)的影响。方法:63例血清Hcy水平增高的高血压患者随机分为2组:(1)对照组(30例):仅服用降压、他汀类及阿司匹林等基础药物;(2)干预组(33例):除基础用药外加用叶酸(5mg/d)、维生素B_(12)(250μg/d)口服。治疗前(基线水平)及治疗后4周,测定患者空腹血清Bcy、NO水平,同时测量患者收缩压(SBP)、舒张压(DBP)及PWV。结果:干预组患者Hcy水平与服药前相比降低(P=0.0000);NO水平与服药前相比升高(P=0.0002);四周后对照组Hcy及NO水平亦有变化但无统计学差异(P>0.05)。两组患者SBP、DBP及PWV基线水平相比无统计学差异(P>0.05);干预组患者SBP、DBP及PWV值与治疗前相比降低均有统计学差异(P=0.0000)。结论:口服叶酸、维生素B_(12)可以显著改善老年高血压伴高Hcy血症患者的血管内皮功能及自主神经功能,有一定的降压及改善血管弹性的作用。Objective:To explore the effects of folic acid and vitamin B12 on tasting serum homocysteine (Hcy), nitric oxide (NO), blood pressure(BP) and pulse wave velocity (PWV)in aged hypertension patients with hyperhornocysteine (HHcy) Methods:63 aged essential hypertension patients with HHcy were included in this study. They were randomly divided into two groups: treatment group (33 cases were given folio acid 5mg/d and vitamin Bu 250μg/d orally in addition to routine tleatment) and control group (30 cases were treated with routine treatment). The blood samples of the two groups were assayed pre-treatment (baseline) and post-treatment(4 weeks later), and the concentrations of Hey and NO were measured and compared. In the meanwhile, systolic blood pressure (SBP), diastolic blood pressure (DBP) and PWV were measured in both groups Results:The Hey concentration of post treatment was lower than that of pre-treatment in treatment group (P=0.000). The NO levels were higher than those of pre-treatment in treatment group (P= 0.003). The difference of Hey and NO is not significant between post- treatment and baseline in control group(P〉0.05). There was no statistical significant difference between the two groups in SBP ,DBP and PWV at baseline level(P〉0.05); After 4 weeks, SBP, DBP and PWV between post-treatment and baseline had significant statistical difference in treatment group(P=0.0000). Conclusion:Folic acid and vitamin B12 could decrease the serum Hey content and increase the NO level in aged essential hypertension patients with HHcy. This therapeutics can improve endothelial function modulate autonomic nervous function.decrease BP and improve arterial elasticity of these patients.
