血清热休克蛋白70在重度子痫前期合并HELLP综合征中的价值探讨

摘　　要：目的:探讨血清热休克蛋白(heat shock protein,HSP)70在重度子痫前期合并HELLP综合征中的价值。方法:用ELISA双抗体夹心法,对22例重度子痫前期合并HELLP综合征患者血清热休克蛋白70含量进行检测,并与140例子痫前期患者和50例同孕龄正常对照维进行比较。结果:轻、重子痫前期组及HELLP组患者血清HSP70含量均明显高于同孕龄对照组(P<0.05,P<0.001,P<0.001);HELLP组血清HSP70含量明显高于同孕龄对照组及轻度子痫前期组,有非常显著性差异(P<0.001);亦高于重度子痫前期组,有显著性差异(P<0.01)。动态观察发现,随着HELLP综合征溶血、肝酶升高及血小板减少的加重和并发子痫抽搐、急性肾功能衰竭、肺感染并胸水、肝被膜下血肿并腹水、胎盘早剥、DIC的发生,血清HSP70水平显著升高,说明HSP70含量随机体损伤加重而增加。结论:血清HSP70含量在重度子痫前期合并HELLP综合征的诊断、疗效观察及衡量疾病严重程度方面具有重要的临床价值。Objective： A probe is made on the value of the serum heat shock protein 70 in the early onset of pre-eclampsia combining into HELLP syndrome. Method： The method of ELISA is used to examine the contents of serum heat shock protein 70 of 22 patients with the early onset of severe pre-eclampsia combing into HELLP syndrome, The results are compared with those of 340 patients with early onset of pre-eclampsia, and those of 50 normal cases in the control group of the same gestadonal age.Results： The contents of serum HSP70 are much higher in both the group with the early onset of severe and mild pre-eclampsia and the group witb HELLP than the control group （P〈0.05,P〈0.001. P〈0.001）.The contents of serum HSP70 in group HELLP are much higher than those of the control group and the group with the early onset of mild pre-eclampsia with a remarkable difference of （P〈0.001）; and still higher than the severe group with a difference of （P〈0.01）. It has been found by dynamical observation that the content of serum HSP70 rises with the occurrence of the hemolysis of HELLP syndrome, the rise of transaminase, convulsions, acute renal failure, an infection of the Imps and hydrothorax, subcapsular b.ematoma of the liver and ascites, placental abruption and DIC due to the deterioration of thrombocytopenia, which indicates the content of HSP70 rises with the severity of body damage. Conclusion： It is of great significance that the content of serum HSP70 can be used in diagnosing, observing the curative effect and measuring the severity of the disease.
