摘 要:目的:探讨子宫颈液基细胞学检查与组织病理学诊断的符合情况,评估子宫颈液基细胞学在妇科普查子宫颈疾病中的价值。方法:对5327例不同年龄人群妇女做子宫颈液基细胞学检查,结果异常者依次做子宫颈多点活检病理学检查。细胞学诊断采用TBS(2001)分级报告系统,阳性诊断包括意义不明的不典型鳞状细胞(ASCUS)及以上标本,所有ASCUS以上病变的患者均有病理组织学诊断结果,对两者进行统计学分析。结果:5327例患者中,门诊、体检的患者阳性率分别为8.59%、5.50%,无显著性差异。子宫颈液基细胞学结果经活检证实的上皮内瘤变主要分布在31~40岁及41~50岁年龄段,鳞癌主要分布在41~50岁年龄段,腺癌高发年龄段为51~60岁。活检阳性结果年龄分布与TCT阳性分布相同。结论:30~50岁妇女应定期采用子宫颈液基细胞学进行妇科体检,可及早发现子宫颈的早期病变。Objective: To explore liquid-based cervical cytology and histopathological diagnosis consistent with the situation and to evaluate the diagnostic value of liquid-based cervical cytology examination in cervical lesions. Methods: 5 327 cases of TCT were observed, in which the positive results were analyzed in different age patients and were compared with the diagnosis of histopathological findings. Cytology diagnosis used TBS (2001) hierarchical reporting system, including the significance of the positive diagnosis of atypical squamous cells of undetermined (ASCUS) and above specimens, all lesions in patients with ASCUS and the above were histopathological diagnosis, then there was statistical analysis. Results: The positive ratio of TCT in out-patients, in-patients and health cheek volunteers was 8.59%, 6.21% and 5.50% respectively, there was no statistical difference. The cervical intraepithelial neoplasia (CIN) were distributed at the age of 31-50, the cervical carcinoma was 41-50, the adenocareinoma was 51-60. The age distribution characteristics of histopathological findings were the same as TCT. Conclusion: The health cheek must be done for women at the age of 30-50 with TCT, in order to find the cervical precancerous lesions.
