延安地区乙型肝炎病毒基因型分布与临床的相关性

摘　　要：目的了解延安地区乙型肝炎病毒(HBV)基因型分布特征,探讨其与慢性HBV感染者的性别、年龄、不同临床疾病谱等的相关性。方法随机选取HBVDNA阳性患者122例,将可检出基因型的121例患者分为慢性无症状携带者(ASC)58例,慢性乙型肝炎(CHB)46例,肝硬化(LC)17例。采用HBV核酸扩增荧光定量PCR检测HBVDNA载量,采用反向杂交技术测定HBV基因型。结果延安地区HBV基因型主要以C型为主。在121例可检出基因型的患者中,C型116例,B型2例,D型2例,B+C混合型1例。结论延安地区HBV基因型主要以C型为主。基因型C主要分布于LC、HBeAg(+)、年龄大于或等于35岁组的患者中。提示基因型C与较严重的肝脏疾病有关。Objective To investigate the distribution of hepatitis B virus （HBV） genotypes in Yan＇an area,and to explore the significance and the relationship of the HBV genotype with gender and age, clinical patterns of chronic HBV infection. Methods 121 serum samples with detected genotype from 122 HBV DNA positive patients with chronic hepatitis B virus infection from Yan＇an area,consisting of 58 chronic asymptomatic HBV carriers （ASC）,46 chronic hcpalitis B （CHB）, 17 liver cirrhosis （LC）, were sub iected to measure HBV DNA in serum using FQ PCR and analyzed by reverse dot blot（RDB）for HBV genotyping. Results In the 121 serum samples with detected genotype, 116 were genotype C, 2 were genotype B, 2 were genotype D, 1 was genotype B＋C. Genotype C was predominant in this region of China. Conclusion Genotype C was the most prevalent in all patient categories in this area. C-enotype C was mainly prevalent in the patients with LC, HBeAg（ ＋ ）and higher age. The results indicate that geno type C is associated with the development of severe liver disease.
