摘 要:目的探讨2型糖尿病(T2DM)患者遗传素质对膳食干预效果的影响,为T2DM患者制定个体化的膳食干预措施提供科学依据。方法随机抽取T2DM新发病例378例,检测其脂蛋白脂酶(LPL)基因S447多态性,根据基因型分为两组,一组为LPLS/X+X/X基因型(A组),另一组为S/S基因型(B组)。然后仅对其进行饮食干预,2年随访结束后,共随访到依从性强的T2DM患者342例(A组60例,B组282例)。检测两组患者的空腹血糖、血脂等指标。比较两组T2DM患者的血糖、血脂水平。结果在膳食干预前两组患者的空腹血糖、血脂等指标间差别无统计学意义(P>0.05);但膳食干预后A组患者的血糖、三酰甘油水平低于B组,而高密度脂蛋白胆固酶高于B组,差别有统计学意义(P<0.01)。结论T2DM人群膳食干预效果受自身遗传素质的影响;膳食干预对LPLS/X及X/X基因型患者的效果较S/S基因型患者明显。Objective To explore the effect of genetic predisposition on dietary intervention for T2DM patients to pro- vide a scientific basis for development of individualized dietary interventions. Methods Three hundred seventy - eight new T2DM patients, randomly selected to determine their lipoprotein lipase (LPL) gene $447 polymorphism, were divided based on genotype into groups A ( LPL S/X + X/X genotype) and B ( S/S genotype). Dietary intervention was performed. The follow - up lasted 2 years. A total of 342 T2DM patients with good compliance ( 60 in group A, 282 in group B ) were followed up. The in- dexes such as fasting blood glucose (FBG) and lipid were detected. Results There was not significant difference in FBG and lipid between 2 groups before intervention ( P 〉 0. 05 ) , but the levels of FBG, lipid were lower in group A than in group B after dietary intervention, the difference was significant ( P 〈 0. 05 ). Conclusion The effect of dietary intervention is affected by ge- netic predisposition in T2DM patients. Dietary intervention effects better in LPL S/X and X/X genotype patients than in S/S.
