逆行带锁髓内钉治疗股骨髁上骨折43例临床观察

摘　　要：目的:探讨逆行带锁髓内钉(GSH)治疗股骨髁上骨折治疗效果。方法:对43例采用逆行带锁髓内钉内固定治疗患者的临床资料(GSH组)与同期应用股骨动力髁螺钉(DCS)治疗股骨髁上骨折43例患者的资料(DCS组)进行对照。结果:GSH组43例骨折愈合41例,占95.35%,延迟愈合1例,占2.33%,骨不连1例,占2.33%;DCS组43例骨折愈合40例,占93.02%,延迟愈合2例,占4.65%,骨不连1例,占2.33%;两组骨折愈合率相比,差异无统计学意义,P>0.05。HSS功能评分结果GSH组优17例,占39.53%,良18例,占41.86%,可5例,占11.63%,差3例,占6.98%。DCS组HSS功能评分结果优18例,占41.86%,良18例,占41.86%,可5例,占11.63%,差2例,占4.65%。两组HSS评分相比,差异无统计学意义,P>0.05。结论:内固定治疗股骨髁上骨折可获得良好的结果。逆行带锁髓内钉内固定治疗股骨髁上骨折可获得良好的结果,具有操作简便、直观、固定牢固等优点。Objective：To study the retrograde intramedullary nail treatment of femoral supracondylar fracture treatment. Methods：43 cases using retrograde intramedullary nail fixation for the treatment of patients with clinical data （GSH group） and earlier application of the femur dynamic condylar screw （DCS） on the femoral condyle fracture of 43 patients, the information （DCS group） were compared. Results：GSH group of 43 cases of fracture healing in 41 cases （95.35%）, delayed healing in 1 case （2.33%）,nonunion 1 cases （2.33%）; DCS group of 43 cases of fracture healing in 40 cases （93.02%）, delayed healing in 2 cases （4.65%）, nonunion in 1 case （2.33%）; groups of fracture healing rate was not statistically significant （P〉 0.05）. HSS function score results of GSH group excellent in 17 patients （39.53%）, good in 18 cases （41.86%）, fair in 5 cases （11.63%） poor in 3 cases （6.98%）. DCS Group HSS function score results were excellent in 18 cases （41.86%）, good in 18 cases （41.86%）, fair in 5 cases （11.63%） and poor in 2 cases （4.65%）. HSS score statistical treatment of the two groups was not statistically significant （P〉0.05）. Conclusion：Fixation for the treatment of supracondylar femoral fractures get good results. Retrograde interlocking intramedullary nail fixation for the treatment of supracondylar femoral fractures get good results, with simple, intuitive, stable fixation and so on.
