中医药辨证治疗腹泻型肠易激综合征多中心随机对照研究

摘　　要：目的验证中医药辨证论治方法治疗腹泻型肠易激综合征(irritable bowel syndrome,IBS)的临床疗效。方法采用随机对照设计,将360例患者随机分为2组,并分别用辨证论治(中药组)和匹维溴胺(西药组)治疗,疗程4周。结果症状疗效评价:治疗后中药组在腹痛程度积分、排便满意度积分、生活干扰积分以及BSS(IBS病情变化积分表)总积分4个方面优于西药组(P<0.01,P<0.05);总体疗效上,中、西药治疗组总有效率分别为93.8%(165/173)和81.3%(143/172),组间差异有统计学意义(P<0.01)。单项症状腹痛的评价:中、西药治疗组总有效率分别为86.1%(149/173)和70.3%(121/172),组间比较,差异有统计学意义(P<0.01);大便性状疗效评价:治疗后中药组在每天排便的最多次数、10天中排便急迫感的天数和Bristol大便性状分型3方面均优于西药组(P<0.01,P<0.05)。结论中医辨证论治IBS-D显示出较好的疗效。Objective To verify the clinical efficacy of Chinese Medicine syndrome-differentiation therapy in treating diarrhea-predominant irritable bowel syndrome IBS-D. Methods With a blinded randomized controlled design adopted, 360 patients with IBS-D were randomly assigned to two groups, the treated group and the control group, they were treated with Chinese medicine and Pinaverium bromide for four weeks respectively. Results Comprehensive evaluation showed that the total effective rate in the treated group was higher than that in the control group significantly （93.8% vs 81.3%, P 〈 0.01 ）. Efficacy assessment on symptoms （by scoring） showed that the efficacy in the treated group was better than that in the control group in aspects of improving abdominal pain （86. 1% vs 70.3% ）, defecation coziness （involving the frequency of defecation, incidence of tenesmus in the latest 10 days and Bristol typing of stool characters）, living interfering, and total BSS score （P 〈0.05 or P 〈 0.01 ）. Conclusion Chinese medicine syndrome-differentiation dependent therapy shows good efficacy in treating IBS-D.
