摘  要:目的研究血管内皮生长因子(VEGF)和TSP-1(thrombospondin1)在病理性瘢痕中的表达及其临床意义。方法应用免疫组化SP法检测正常皮肤、扁平瘢痕、增生性瘢痕和瘢痕疙瘩组织中VEGF、TSP-1蛋白的表达并进行统计学分析。结果病理性瘢痕组织中VEGF蛋白表达增高,与正常皮肤、扁平瘢痕对照组比较差异有统计学意义(P〈0.05);病理性瘢痕组织中TSP-1蛋白表达显著低于正常皮肤、扁平瘢痕对照组(P〈0.05);四类组织中VEGF蛋向和TSP-1蛋白呈负相关(P〈0.05)。结论VEGF和TSP-1的表达与病理性瘢痕形成机制有关,VEGF可能通过诱导血管生成而促进瘢痕增生,TSP-1降低导致病理性瘢痕中血管的增生,并可能抑制VEGF的升高,从而导致病理性瘢痕的形成。Objective To study the expression of VEGF and TSP-1 ( thrombospondin 1 ) in the pathological scars and its significance. Methods Immunohistochemiscal technique was adopted to detect the expression and distribution of VEGF and TSP-1 in hypertrophic scar (30 cases), keloid ( 18 cases), normal scar (22 cases) and normal skin (23 cases) and statistics was used to analyze the data. Results The expression levels of VEGF were significantly higher in keloids and hypertrophic scars than those in normal sears and normal skins ( P 〈 0.05 ) , while VEGF protein expression was not statistically different between the hypertrophic scar and keloid ( P 〉 0.05 ) , the expression levels of TSP-1 were significantly lower in keloids and hypertrophic scars than those in normal scars and normal skins ( P 〈 0. 05) , while TSP-1 protein expression was not statistically different between the hypertrophic scar and keloid ( P 〉 0. 05 ) , there was a negative correlation between VEGF and TSP-1 in scars. Conclusion The result indicated that VEGF and TSP-1 played important roles in the pathogenesis of pathological scar, the VEGF could promote the formation of pathological scar by inducing angiogenesis of pathological scars. TSP-1 could inhibit angiogenesis of pathological scars.
