经阴道超声诊断子宫内膜息肉的临床价值

摘　　要：目的探讨经阴道超声诊断子宫内膜息肉的临床价值。方法分析175例经阴道超声诊断为子宫内膜息肉患者的声像图特征，并分析宫腔镜检查及病理诊断符合情况。结果175例患者经病理检查证实为子宫内膜息肉者142例，经阴道超声诊断符合率为81．14％（142／175），误诊率为18．86％（33／175）。其中，对不同声像图特征的诊断符合情况分析：提示单发子宫内膜息肉，直径小于1cm者其诊断符合率为83．33％（20／24）；单发，直径在1～2cm者为92．31％（60／65）；单发，直径大于2cm者为50．00％（4／8）；多发者为74．36％（58／78）；经宫腔镜检查诊断符合率为91．61％（142／155），误诊率为8．39％（13／155）。结论阴道超声检查诊断子宫内膜息肉准确率高，具有无创性及经济简便的优点，可作为子宫内膜息肉的首选筛查手段。Objective To evaluate clinical value of transvaginal uhrasonography （TVS） in diagnosis of endometrial polyp （EP）. Methods The sonographic characteristics of 175 women with EP diagnosed by TVS were analyzed and accordance rate of TVS with pathological results was compared with accordance rate of hysteroscopy（HS） with pathologial results in diagnosis of EP. Results Among 175 cases of EP diagnozed by TVS, 142 cases were confirmed by pathological examination, the accordance rate was 81. 14% （142/175） and the misdiagnosis rate was 18. 86% （33/175）. Of which, the accordance rates for diffterent sonographic characteristics showed that the accordance rates of TVS for single polyp with a diameter less than lcm was 83.33% （20/24） , that for single polyp with a diameter between 1 - 2cm was 92.31% （60/65）, that for single polyp with a diameter larger than 2cm was 50.00% （4/8） and that for multiple polypi was 74.36% （58/78）. While the accordance rate of HS for EP was 91.61% （ 142/155 ） and the misdiagnosis rate was 8.39% （ 13/ 155 ）. Conclusion TVS has a higher accuracy rate in diagnosis of EP. Because TVS is non-invasive, economic and simple, so it becomes the first-chosen screening method of EP.
