摘　　要：高危妊娠是导致孕产妇死亡的主要原因，也是孕产期保健管理工作的重点和难点。高危妊娠的发生与发展存在着一些共性特点。针对高危妊娠管理中的突出问题，保健人员与产科医生在长期的实践与研究中积累了丰富的管理经验。近年来，国外在对高危妊娠的研究中取得了一些新的进展，加深了对孕产妇疾病与死亡的认识，提出“严重急性孕产妇病状（孕产妇临近死亡）”的概念，强调树立产科重症监护意识，建立专门针对危重孕产妇的产科重症监护室，此外还探讨了高危妊娠筛查方法，建议用孕期疾病的表现特征筛查需要重点关注的健康问题，为进一步完善高危妊娠保健管理提供了新的思路。High-risk pregnancy is the main cause of death of pregnant and lying-in women, which is also emphasis and difficulty in maternal health management. Domestic studies show that there are some common characteristics in occurrence and development of high-risk pregnancy. Aiming at major problems in high-risk pregnancy management, healthcare workers and obstetricians have done many researches and acquired lots of management experiences in long-term practice. Recently, foreign researches on high-risk pregnancy have made some progress. A new conception called ＂severe acute maternal morbidity （ maternal near miss） ＂ has been proposed on the basis of further understanding of maternal morbidity and mortality, on which consciousness of obstetric intensive care needs to be strengthened and obstetric intensive care unit should be established for those pregnant women with critical diseases. Besides, they explored the screening methods of high-risk pregnancy and recommended that important health problems in pregnancy should be screened by utilizing manifestation characteristics of the diseases during pregnancy in high risk pregnancy screening. These progresses provide new thoughts for further improvement in health management for high-risk pregnancy.
