超声心动图和双源CT诊断川崎病冠状动脉瘤的初步对比研究  

Comparison Study of Echocardiography and Dual-source CT in Diagnosis of Coronary Artery Aneurysm Due to Kawasaki Disease

摘　　要：目的探讨超声心动图和双源CT在川崎病冠状动脉瘤的诊断价值,并将2种技术检查结果进行对比分析。方法收集在我院行超声心动图及双源CT检查的10例川崎病患儿,观察冠状动脉的扩张程度和冠状动脉瘤的形态、大小、位置和数目。结果超声心动图显示所有患儿左、右冠状动脉均扩张,内壁粗糙,共有21个冠状动脉瘤形成,5例患儿扩张的瘤内可见附壁血栓,部分血栓可见钙化。双源CT、共观察到29个冠状动脉瘤,其中8个较小或位于中远段的冠状动脉瘤超声心动图末能发现。结论彩色多普勒超声心动图具有安全、简便、准确、可靠的优势,并能重复动态观察冠状动脉扩张及其演变的全过程,但对冠状动脉远段的病变无法检出,而双源CT可与超,杆心动图互补,图像与心血管造影图像很相似,已成为无创伤性的新技术。Objective To assess the value of color Doppler flow in diagnosing coronary artery aneurysm of Ka wasaki disease and compare it with Dual-source CT （DSCT） . Methods A total of 10 cases hospitalized in our hospital were reviewed retrospectively. Coronary artery aneurysm of Kawasaki disease was diagnosed by echocardiography and DSCT. The number, location, shape and size of each coronary artery aneurysm gotten from DSCT were observed and compared with those from echocardiography simultaneously. Results The characteristics of 2-D echocardiogram findings included dilated left and right coronary artery, formed twenty one coronary artery aneurysms, rough endometri urn and thrombosis of coronary artery. There were thrombosis and calcification in part of the thrombosis in five patients. Twenty-nine coronary artery aneurysms were observed by DSCT. Echoeardiography failed to detect 8 aneurysms of small size and located in the middle or distalsegments of the coronary Arteries. Conclusions Echocardiography is safe, feasible, accurate and reliable. It can repeatedly obtain dynamic observation of the whole course of cor onary artery dilation and succession, hut it fails in diagnosing the distal segments of coronary diseases. As a non invasire imaging modality, DSCT can make up for the shortcoming of echocardiography and its image is similar to coro nary angiography.
