动物解剖实验-硬脊膜下腔的客观存在

摘  要:目的通过对实验动物解剖,以及对2例患者资料分析,为临床工作特别是临床麻醉中所遇到的异常阻滞平面提供佐证,提醒麻醉医师在工作中注意硬脊膜下腔的存在,及时认识到某些临床症状根源于硬脊膜下腔阻滞,而尽早采取正确的应对措施,尽可能避免出现由于硬脊膜下腔阻滞所引起的严重危险性。方法解剖离体牛脊柱和解剖活体狗脊柱,同时分析以往临床中出现广泛阻滞2例患者的影像学资料。结果离体牛脊柱上硬脊膜下腔较易分离,暴露完全;活体狗的硬脊膜下腔基本暴露完全,亦可证实硬脊膜下腔的客观存在性;临床患者术后X线征象表明硬膜外导管位于硬脊膜与蛛网膜之间的腔隙-硬脊膜下腔。结论从动物解剖学和临床角度分析2例患者资料,表明硬脊膜下腔的客观存在性。Objective To analyze the anatomy of subdural space can make an evidence to the exceptional level of anesthesia in clinic working, which reminds anesthetists to pay attention to the presence of subdural space and to realize that some complications may root in the subdural block, therefore to avoid the serious and dangerous complication following extradural block. Methods Dissecting both on the fresh cow lumbar spine and on the living dog spine to find the subdural space, and retrospectively reviewing the radiological informations of the two patients who had extensive subdural blockage. Results The subdural space can be seperated easily and also can be exposed completely in the fresh cow spine. The subdural space can be seperated in most of the live dog. The radiological information of patients who had the exceptional level of anesthesia in the operation proved that the epidural catheter is neither in the epidural space nor in the subarachnoid cavity,but in another space named subdural space. Conclusion The subdural space is in existence instead of the dissecting effect of the puncture needle. The anatomy characteristic of subdural space can make a reasonable explanation to the exceptional level of anesthesia in clinical work.
