基层医院应用PPH术治疗重度痔病60例临床观察

摘　　要：目的探讨60例重度痔病患者行PPH术(痔上粘膜吻合器环切术)临床疗效。方法对我院自2008年2月至2009年5月住院治疗的60例重度痔病患者,用国产的PPH吻合器,行痔上粘膜环切术进行回顾性分析。结果手术时间为11～30 min,平均18.37 min,术后3～7天出院,平均5.5天。随访所有病人临床症状消失,肛查见肛缘痔核消失,肛门镜检见内痔核全部消失,无1例严重并发症和后遗症,1例出现术后出血需要局麻下再次手术8字缝合止血。结论PPH术是一种操作简单、手术时间短、出血少、痛苦小、恢复快、疗效好、安全可靠的一种治疗重度痔病的新术式。国产PPH吻合器消毒后可重复使用,降低总体费用,效果也令人满意,减轻患者经济上的负担,能在基层医院大量推广。Objective To investigate the clinical effect of PPH method in the treatment of severe hemorrhoids. Methods From February 2008 to May 2009, 60 cases with severe hemorrhoids received PPH were analyzed retrospectively. Results The operation and hospitalization time were 11- 30 minutes and 3-7 days respectively. After treatment, all the hemorrhoids were disappeared completely. Reoperative hemostasis was performed in one ease because of postoperative anastomotic bleeding. Conclusion PPH is an effective measure for the treatment of the severe hemorrhoids. Its advantages are shorter time of operation, quicker recovery, less complications and a lower recurrence rate. The domestic-made metal stapler can be used repeatly and the hospitalization costs are cheaper. So PPH can be popularized in primary hospitals.
