不同剂量的吉西他滨和顺铂治疗老年晚期非小细胞肺癌临床观察

摘　　要：目的探讨低剂量的吉西他滨和顺铂治疗老年晚期非小细胞肺癌的疗效及毒副作用。方法老年晚期NSCLC患者58例,随机分为正常剂量组30例和低剂量组28例,正常剂量组予吉西他滨(1000mg/m2)+顺铂(80mg/m2)方案,低剂量组予吉西他滨(600mg/m2)+顺铂(30mg/m2)方案,用药方法2组相同。结果正常剂量组的近期有效率和疾病控制率分别为40.0%和83.3%,低剂量组的近期有效率和疾病控制率分别为39.3%和82.1%,2组患者近期疗效相比差异无显著性。2组患者化疗毒副作用比较,低剂量组的白细胞及血小板减少较正常剂量组差异有显著性(P<0.05);Ⅲ～Ⅳ度毒副作用,正常剂量组为23例,低剂量组为7例,2组间比较差异有显著性(P<0.05);其余两组间比较差异无显著性(P>0.05)。结论对老年晚期NSCLC患者,采用低剂量吉西他滨联合低剂量顺铂治疗具有一定优势,值得临床上进一步观察与探讨。【Objective】To evaluate the curative effect and toxicity of different dose Gemcitabine and cisplatin in the treatment of elderly patients with advanced non-small cell lung cancer.【Methods】The patients with NSCLC were divided into two groups：randomly normal-dose group and low-dose group,normal-dose group treated with Gemcitabine （1000 mg/m^2） and Cisplatin （80 mg/m^2）,and low-dose group treated with Gemcitabine （600 mg/m^2） and Cisplatin （30 mg/m^2）,the other treatments were the same.【Results】The RR and DCR of normal-dose group were 40.0% and 83.3%,and those of low-dose group were 39.3% and 82.1% respectively,there were no significance between the two groups.For the side effects,patients with low leucocyte or thrombocytopenia,and Ⅲ～Ⅳside effect in low-dose group were fewer than that of the normal-dose group,there were significance between the two groups.【Conclusion】For elderly patients with advanced non-small cell lung cancer,treated with low-dose of Gemcitabine and Cisplatin have certainly preponderance,it is worthy the further study.
