摘  要:目的:观察血管钠肽对皮瓣碾压撕脱后血管内皮细胞通透性的影响。方法:39只SD大鼠用皮肤碾压撕脱伤模型机制作背部3cm×9cm大小碾压撕脱皮瓣后随机分为生理盐水1ml/kg腹腔注射组,血管钠肽0.1mg/kg静脉注射组,地塞米松5mg/kg腹腔注射组,每组13只动物。各组给药时间点为术后2h、1天、2天和3天。大鼠的血清于术后12h、1天、2天和3天收集,用以体外内皮细胞通透性改变的检测。术后7天观察皮瓣成活面积。结果:各组皮瓣成活面积:生理盐水组37.42%±4.36%,血管钠肽组49.96%±2.32%,地塞米松组43.16%±4.13%;各组内皮细胞通透性改变:生理盐水组(6.65±0.09)μg/ml,血管钠肽组(5.50±0.11)μg/ml,地塞米松组(5.95±0.09)μg/ml。血管钠肽用药组术后7天皮瓣成活面积明显高于其它两组,内皮细胞通透性也较其它两组低,差异有统计学意义(P<0.05)。结论:血管钠肽可以降低内皮细胞的通透性进而增加碾压撕脱皮瓣的成活面积。Objective To observe the impact of vasonatrin peptide on the endothelial permeability after skin flap was crush-avulsion injured. Methods 39 SD rats were randomly divided into 3 groups with each group 13 animals. A 3cm × 9cm caudally based dorsal random flap was elevated and then our self-made model machine was used to produce the crush-avulsion flap. Group A was given normal saline lml/kg i.p., Group B was given vasonatrin peptide 0.1mg/kg i. v., Group C was given dexamethasone 5mg/kg i.p. and the time point of administration were 2h, 1d, 2d, and 3d after operation. The blood serum of the rats was collected12h, 1d, 2d and 3d after operation and it was used as inducer to determine the impact of vasonatrin peptide on the endothelial permeability. Skin flap survival rate was calculated on the seventh postoperative day. Results Flap survival rate in each group was as follows: group A, 37.42±4.36 percent; group B, 49.96±2.32 percent; group C, 43.16±4.13 percent. A statistically significant increase in flap survival was observed in group B as compared with group A and group C. The difference between all groups was significant (P〈 0.05). The endothelial permeability was (6.65±0.09)μg/ml, (5.50±0.11)μg/ml, (5.95±0.09)μg/ml, respectively. There was significant difference statistically. Conculsion Vasonatrin peptide can decrease endothelial permeability and increase avulsed flap survival.
