老年巨大前列腺手术中生物蛋白胶的应用

摘　　要：目的观察医用生物蛋白胶在60mL以上巨大前列腺开放摘除术中止血效果的比较。方法采取耻骨上经膀胱横向弧形切口暴露前列腺，环绕颈口及前列腺外科包膜下注射医用无水酒精，剜出腺体后，热盐水纱布填塞腺窝3～5min，连续锁边缝合完成“水滴式”颈口成形，试验组（A组）向腺窝内喷洒生物蛋白胶，共35例；对照组（B组）手术及腺窝缝合方法与A组完全一样，仅不使用生物蛋白胶。结果A组与B组比较，在术后腺窝（尿道外口）渗血[（6．0±1．5）h vs．（12．8土2．4）h]、冲洗液转清[（8．5±2．1）hvs（22．0±3．O）h9、导尿管牵拉时间[（12．0±2．5）hvs（26．o±3．2）h3、拔管时间[（76．0±5．1）hvs（133．0±10．3）h]、住院天数[（8．0±2．8）dvs（13．0±2．2）d35个指标上两组差异均有显著性意义（P均〈0．05）。结论巨大前列腺开放切除手术中，辅助应用生物蛋白胶止血，效果确切，能明显减少术后出血，促进恢复，副作用少，值得临床应用。Objective To observe hematischesis effect of the medical biological protein adhesive in above 60mL open prostate gland excision. Methods The prostate gland was exposed with bladder crosswise arc incision. The medical dehydrated alcohol was injected around the encirclement diameter mouth and under the prostate gland. After the gland was taken out, the hot salt water gauze pads were put into the gland nest for 3~5 minutes. Then continual border closing sutures completed ＂the water-drop type＂of the throat opening. Biological protein adhesivewas sprayed to the gland nest of the experimental group （A group,35 cases）. The control group （B group） received the same surgery and the gland nest sutures as A group, but did not use the biological protein adhesive. Results A group had oozing of blood for（6.0 ±1.5）h, the rinse solution became clear after （8.5 ± 2.1）h, the catheter was inserted for（12.0 ± 2. 5）h and taken out after（76.0 ± 5.1 ）h,hospital stay was（8.0 ± 2.8）d. All indexes were better than B group obviously. Conclusion In the open prostate gland excision surgery, the hematischesis effect of biology protein adhesive is obvious. It can reduce bleeding and help recovery with fewer side-effects.
