血管内介入治疗96例颅内动脉瘤临床分析

摘　　要：目的:探讨血管内介入治疗动脉瘤的临床应用价值。方法:回顾分析96例动脉瘤病人的临床资料,对血管内介入治疗动脉瘤的临床效果作出评价。结果:100个动脉瘤,86个行弹簧圈栓塞术,完全栓塞62个(69.8%),次全栓塞22个(25.6%),不完全栓塞2个(2.3%)。发生术中动脉瘤破裂3例(3.5%),脑血栓形成或脑梗死3例(3.5%),血管痉挛2例(2.3%)。术后动脉瘤复发2例,未见动脉瘤栓塞后再出血,脑积水5例,颅内感染1例。病人出院12mo后完全恢复正常生活、工作者65例(72.2%);有轻度到中度神经功能障碍10例(11.1%);有严重神经功能障碍及死亡者5例(5.6%)。结论:血管内介入治疗是一种安全有效的治疗动脉瘤的方法。Objective：To investigate the clinical value of endovascular interventional treatment for intraeranial aneurysm. Methods：The clinical data of 96 cases with intracranial aneurysm was analyzed retrospectively and the treatment effect of endovascular interventional therapy in intraeranial aneurysm was evaluated. Results： In 100 intraeranial aneurysms, 86 underwent the embolism with intravascular coil, we completely embolized 62 cases （69.8%） ,imperfectly embolized 22 eases（25.6% ） ,and inadequately embolized 2 cases （2.3%）. 3 eases of aneurysms （3.5%）ruptured during the embolization, 3 patients （ 3.5 % ） had cerebral infarction or thrombosis, 2 cases （2.3 % ） occurred cerebral vasospasm. After operation, recanalization of aneurysm occurred in 2 cases, no one hemorrhaged again, 5 cases had hydrocephalus, one occurred intracranial infection. 90 cases （93.7%） were followed up 1 - 2years. The modified rankin scale of patients with aneurysm was used to evaluate quality of life after leaving hospital 12 months, 65 patients （72.2%） can work and live normally, 10 cases（ 11.1% ） have feeble and middle nerve functional disturbance, and 5 cases （5.6%） have severe nerve functional disturbance or died. Condusion：Endovascular interventional treatment for patients with intracranial aneurysm is an effective and salty method.
