彩色多普勒超声检测Graves病患者甲状腺动脉血流的变化

摘　　要：应用彩色多普勒超声检查Ｇｒａｖｅｓ病３０例及治疗后缓解２６例，并以１２例正常人作对照。发现：右下动脉检出率治疗前显著高于对照，并且在三组间有差异。左右上动脉Ｖｍａｘ和Ｖｍｉｎ治疗前组较对照显著增高，且较缓解组增高；左右上动脉Ｖｍａｘ缓解组较对照增高，而Ｖｍｉｎ缓解组与对照无差异。右下动脉Ｖｍａｘ和Ｖｍｉｎ治疗前组较对照高，但治疗前组与缓解组无差异，缓解组与对照无差异。提示：（１）、右下动脉检出率高为ＧＤ治疗前的特征；（２）、治疗前Ｖｍａｘ和Ｖｍｉｎ均明显增高，表明甲状腺处于高血流状态，有助于诊断。ＧＤ时甲状腺高血流的原因有待进一步探讨；（３）、缓解组左右上动脉Ｖｍｉｎ和右下动脉Ｖｍａｘ和Ｖｍｉｎ随甲状腺激素水平的恢复而较早恢复，对疗效评价意义较大。而缓解组左右上动脉Ｖｍａｘ滞后于甲状腺激素水平的恢复。Thirty Graves disease patients(GD)and 26 remission patients were examined by directional color Doppler ultrasound.Only 4 patients suffering GD remained.The inferior right thyroid artery detective rate was significantly higher than the control.The maximum blood flow velocity(Vmax)and minimum blood flow velocity(Vmin)of the patients superior right and left arteries were significantly higher than control,also significantly higher than the remission ones.Vmax of remission patients was higher than control.Vmax and Vmin of inferior right artery of the patients were significantly higher than control,but in remission patients were not higher than control.The results showed that higher detective rate of right inferior artery was the unique change in the GD patients.Color Doppler ultrasound may detected high blood flow in the GD patients.Vmin of superior right and left arteries and Vmax and Vmin of inferior right arteries were simultaneously recovered of the thyroid hormone in the remission patients.Vmax of superior right and left arteries were later recovered of its thyroid hormone level in remission patients.
