能量合剂辅助治疗急性有机磷农药中毒中间综合征临床研究

摘　　要：目的观察能量合剂辅助治疗急性有机磷农药中毒中间综合征(IMS)的临床效果。方法120例IMS中毒的患者随机分为治疗组64例和对照组56例。对照组常规应用氯磷定、阿托品及机械通气,治疗组在对照组基础上应用纳洛酮、能量合剂,观察上述药物的用量、自主呼吸恢复时间及血胆碱酯酶活力(CHE)和血浆肿瘤坏死因子(TNF-α)的水平变化,及2组患者的治愈率。结果治疗组患者较对照组的机械通气持续时间及自主呼吸恢复时间缩短,出现IMS至完全恢复自主呼吸期间阿托品用量减少氯磷定用量增多,治愈率提高,同时患者的CHE、TNF-α水平的恢复优于对照组,各项比较差异均有统计学意义(P<0.05)。结论能量合剂可以提高有机磷农药中毒中间综合征患者的治愈率,减少药物应用剂量,同时有助于改善血浆CHE、TNF-α水平。Objective To observe the clinical effects of energy compoand on the treatment of acute organophosphorus pesticide poisoning complications intermediate syndrome（IMS）.Methods 120 cases of IMS were randomly divided into therapy group（n=64）and control group（n=56）.Therapy group were treated with energy compound combined with atropine and PAM and invasive mechanical ventilation,and control group were treated with atropine and PAM and invasive mechanical ventilation.The effects were compared.observed the usage of these drugs,self-breathing recovery time and blood cholinesterase activity（CHE） and plasma levels of tumor hecrosis factor（TNF-α）.Results Compared with control group,the therapy group medchanical ventilation time and spontaneous breathing recovery time shortened;Appears IMS to fully revove spontaneous breathing during the dosage of atropine reduced,PAM incrcased;The cure rate increased;CHE,and TNF-α were better recovery.All the indicators were statistically significant difference between the two groups（P〈0.05）.Conclusion Energy compound has great clinical effects on the treatment of IMS and the decrease of the mortality.
