异体手指移植修复拇指Ⅵ°缺损

摘　　要：目的：评估异体手指复合组织移植修复拇指Ⅵ°缺损的临床效果。方法：用－３０℃冷藏的健康青年尸手指骨—关节—肌腱—腱鞘复合组织为拇指支撑架，缝接供、受体肌腱，包绕附有趾骨片的甲皮瓣。结果：４２例中３８例异体骨与宿主骨性连接，皮肤感觉恢复，对掌功能恢复３０例，其中优良级２１例。但异体关节软骨变性，部分病例异体骨有吸收现象。结论：自体甲皮瓣包绕冷冻异体手指复合组织移植，能恢复拇指Ⅵ°缺损的长度，皮肤覆盖与手指外形满意。Objective:to evaluate clinical effect of thumb reconstruction with composite tissue allograft on Ⅳ degree thumb defect.Methods:The support of thumb was reconstructed by phalanx joint tendon sheath composite tissue,which was harvested from fresh cadavers of healthy young persons and preserved at 30℃ as bank storage.The allografts were wrapped by skin nail flap of hallux inlaying a piece of autogenous vascularized phalanx.Results:Bony union between the impacted host bone and the allogeneic phalangeal bone was seen on X ray films in 38 of 42 cases.The sensory function of thumbs is satisfactory.The opposition function examined in 30 cases and of 21 cases are in excellent and good results.Bone absorption or joint destruction occurred in some cases after transplantation.Conclusions:The frozen composite tissue allograft with skin nail flap of hallux can be selected to reconstruct thumb.Its cosmetic appearance and function were better than others for repairing Ⅳ degree thumb defect.Bone absorption or joint destruction could be prevented by inlaying to each allogeneic phalanx a piece of autogenous vascularized phalanx with the free hallux flap.
