摘  要:目的:探讨应用实时三维超声成像技术在胎儿体表畸形诊断方面的应用价值。方法:应用实时三维超声成像技术及二维超声对8 025例胎儿进行产前超声检查,分析二维及实时三维超声成像提供的畸形部位、形态改变的准确性及直观性,统计并比较二者超声诊断与产后诊断的符合率。结果:①发现胎儿体表畸形45例66处。二维超声诊断与产后诊断相符51处(符合率77.27%),实时三维超声成像诊断60处(符合率90.91%),二者比较差异有统计学意义(P<0.05)。②实时三维超声成像可直观显示胎儿体表结构异常特征、对立体形态及各结构部位的关系,并实现了对胎儿运动的立体图像的实时观察,提高了其诊断的准确性。结论:实时三维超声成像图像立体、直观,为胎儿体表畸形的诊断提供了更丰富的信息,优于单纯使用二维超声,是目前产前诊断可靠的技术手段。Objective: To explore the application value of real - time three - dimensional uhrasound in evaluation of fetal surface malformations. Methods: 8 025 fetuses were examined by real - time three - dimensional ultrasound and two - dimensional ultrasound, the positions of fetal surface malformations, accuracy and direct - viewing of morphological changes produced by real - time three - dimensional ultrasound and two - dimensional ultrasound were compared, the coincidence rate of ultrasound diagnosis and postpartum diagnosis was analysed. Results: 45 fetuses (66 positions) were diagnosed as fetal surface malformations, the coincidence rate of two -dimensional ultrasound diagnosis and postpartum diagnosis was 77.27% (51 positions) , and the coincidence rate of real - time three - dimensional ultrasound and postpartum diagnosis was 90. 91% (60 positions) , there was significant difference (P 〈 0. 05 ) . Real - time three - dimensional ultrasound displayed abnormal characteristics of fetal surface malformations, the relationship between three - dimensional shapes and structures, achieved real - time observation on three - dimensional images of fetal movements, increased diagnostic accuracy. Conclusion : Real - time three - dimensional ultrasound is tridimensional and intuitionistic, whieh produces more sufficient information for diagnosis of fetal surface malformations and is superior to two - dimensional ultrasound and a reliable technique in prenatal diagnosis.
