摘 要:目的:研究慢性心力衰竭肾阳虚证患者的代谢模式,探讨代谢组学在中医证候学研究中的应用。方法:收集慢性心力衰竭患者54例(其中肾阳虚证27例、非肾阳虚证27例)及正常人24例的空腹晨尿尿样,通过气相色谱/质谱(GC/MS)联用及模式识别为主的代谢组学方法进行研究。结果:主成分分析和正交最小二乘法分析中慢性心力衰竭肾阳虚证组、非肾阳虚证组和正常组的代谢成分积分在空间分布上明显分开,且样本均投影到2SD(标准差)的区域内,表示其值可信,说明所建模型能准确区分3者。慢性心力衰竭患者与正常人比较还发现了柠檬酸、丙氨酸、3-甲基戊烯二酸、丙胺、组胺5种差异性代谢物质。结论:慢性心力衰竭肾阳虚证患者、非肾阳虚证患者和正常人具有不同的代谢模式,以GC/MS为技术特点的代谢组学研究方法能将3者区分。代谢组学是研究中医学证本质的有效方法。Objective: To investigate metabolic patterns of chronic heart failure with kidney-yang deficiency and discuss the application of metabolomics in study of TCM syndrome. Methods: The urine samples of 24 healthy volunteers and 57 patients with chronic heart failure were studied (kidney yang deficiency 27 and non-kidney yang deficiency 27) through multiples bioinformatics processing technology, including metabolomics method, chromatography-mass spectrometry technique, as well as complex chemical, biology pattern recognition and data mining. Results: Principal components analysis and orthogonal partial least square discriminant analysis found that singulorum sample metabolism principal constituent score spot spatial distribution indicated kidney-yang deficiency, no kidney-yang deficiency and normal control had significant difference. Singulorum sample metabolism principal constituent score less than 2 times the standard deviation, the results showed that the model can distinguish kidney-yang deficiency and non-kidney-yang deficiency.Chronic heart failure group compared with the control group, displayed significant changes in citric acid, alanine, hmgcoa, propylamine, histamine. Conclusion: Metabolomic pattern of kidney yang deficiency and non-kidney yang deficiency were different. Kidney yang deficiency and non-kidney yang deficiency can be distinguished by multiples bioinformatics processing technology. Therefore, metabolomics can provide a new space for the development of TCM syndrome.
