摘  要:目的:观察650nm-10.6μm复合激光治疗膝骨关节炎患者的临床疗效。方法:56名患者被随机分为复合激光组与发光二极管组(每组各28例),接受为期6周共14次的治疗。取双侧犊鼻穴,激光组采用650nm-10.6μm复合激光照射,对照组采用发光二极管红光照射。本研究为随机、双盲、对照临床试验,采用西安大略和麦克马斯特大学骨关节炎调查量表(WOMACVA3.1)问卷进行疗效评价。并对患者疗效自评进行统计分析。结果:两组患者一般情况指标及治疗前WOMAC疼痛、僵硬、功能得分差异无显著性意义(均P>0.05),具有可比性。治疗2周、6周及治疗结束4周后,两组患者WOMAC疼痛、僵硬、功能得分均显著降低(均P<0.01);复合激光组治疗2周、6周及治疗结束后4周WOMAC疼痛、僵硬及功能的改善率优于发光二极管组,但未达统计学显著水平(均P>0.05);两组患者对疗效的自评差异无显著性意义(P>0.05)。结论:650nm-10.6μm复合激光穴位照射对膝骨关节炎患者膝关节疼痛及功能障碍有显著疗效。但因样本偏小,尚不能明确复合激光照射的这一疗效是否优于发光二极管红光照射,应在此初步研究基础上,进一步开展大样本临床研究。Objective: To evaluate the effects of 650nm- 10.6μm compound laser on patients with knee osteoarthritis (OA). Methods: Fifty-six patients with knee OA were randomly divided into compound laser group and RLED group (28 per group), receiving a 20-minute irradiation with 650nm-10.6μm compound laser or red light emitting diode respectively on acupoint Dubi (ST35) for 6 weeks, 14 times in all. This was a randomized, double-blinded, placebo-controlled clinical study; main outcome measures were Western Ontario and McMaster Universities' Osteoarthritis Scale (WOMAC VA3.1) scores. In addition, patients' global assessment was analyzed. Results: No difference of general data of patients was found in between-group comparison before treatment (P〉0.05), either of WOMAC scores (pain, stiffness and function, respectively) were found in between-group comparison (all P〉0.05). The WOMAC scores (pain, stiffness and function, respectively) of both goups reduced significantly compared with baseline after 2 or 6 weeks treatment and 4 weeks after treatment (all P〈0.01); Though there was no significant difference of WOMAC scores (pain, stiffness and function, respectively) between two groups (all P〉0.05), the reduce rate of the compound laser group was better than that of the RLED group. Neither the patients' global assessment showed statistical differences between two groups (P〉0.05). Conclusion: 650nm-10.6μm compound laser was beneficial to patients with knee OA. But there was no statistical differences between the compound laser and the red light irradiation. Further study and a larger samplesize were required.
