摘 要:【目的】探讨Sky骨扩张器在老年(61~90岁)骨质疏松性椎体压缩骨折椎体后凸成形术(PKP)中应用的临床效果。【方法】自2005年3月至2008年3月对32例共42个椎体压缩骨折老年患者,应用Sky骨扩张器系统进行经皮穿刺、塌陷椎体扩张后注入骨水泥。随访观察患者疼痛视觉模拟评分(VaSklalarialoguescale,VAS)及并发症情况。【结果】42个椎体经重侧或双侧椎弓根穿刺成功完成手术,手术时间30~120min,骨水泥注入量每个椎体3.0~6.8mL,骨水泥返流入椎旁静脉1个椎体,均无临床症状,无椎管内漏及椎间隙漏等并发症。随访12~26个月。所有患者疼痛缓解,VAS术前平均为(7.6±0.7)分,术后d。平均为(3.5±0.4)分,d,平均(2.8±0.5)分,术后1个月平均(2.4±0.5)分。手术前后有显著差异(P〈0.05)。【结论]Sky骨扩张器应用于椎体后凸成形术能达到消除疼痛、恢复椎体高度的目的,是治疗老年性骨质疏松性压缩骨折的有效方法。[Objective]To explore the clinical efficacy of Sky bone expander system in percutaneous kyphoplasty for the treatment of osteoporotic vertebral compression fracture. [Methods] Thirty two cases(aged 61~90, 42 vertebrae) underwent percutaneous kyphoplasty by using Sky bone expander system from March 2005 to March 2008. The bone cement was injected into the collapsed vertebrae. The visual analogue scale (VAS) and complications were recorded during follow up. [Results] All cases were successful experienced the procedure by using puncture of unilateral or bilateral neck of vertebrae. The operation time ranged from 30 to 120 minutes. The mean volume of cement injected into each vertebra body was 3.0 to 6.8ml. Extra-vertebral leakage of bone cement was observed in two vertebrae with no clinical symptom. The symptoms of the patients were improved during the follow up of 12--26 months without recurrence. All patients had successful intervention, and the visual analogue scale (VAS) was 7.6±0.7 before operation, 3.5±0.4 at one day and 2.8± 0.5 at one week after operation, 2.4±0.5 at one month after operation, with significant difference between pre-operation and post-operation ( P〈0.05). [Conclusion] Application of Sky bone expander system in percutaneous kyphoplasty can relieve pain and restore the vertebrae height, and it may be an effective treatment for aged patients with osteoporotic vertebral compression fracture.
