小剂量氯胺酮伍用丙泊酚混合液对丙泊酚注射痛的影响

摘　　要：【目的】观察小剂量氯胺酮伍用丙泊酚混合液对丙泊酚注射痛的影响。【方法1120例ASAⅠ～Ⅱ级择期手术实施气管插管全身麻醉患者，随机分为三组，每组40人。Ⅰ，Ⅱ组患者在注射1．5～2mg／kg异丙酚前1min分别注射2mL生理盐水、2mL100μg／kg氯胺酮，Ⅲ组患者注射与其相同剂量氯胺酮和异丙酚的混合液。应用注射异丙酚时的VRS评分和苏醒后VAS评分法分别对各组预防异丙酚注射痛的效果进行评价。【结果】①VRS评分：与Ⅰ组相比，Ⅱ、Ⅲ组患者无痛发生率明显升高（P〈0．05），Ⅱ和Ⅲ组之间差异无统计学意义（P〉0．059；②VAS评分：与Ⅰ组相比，Ⅱ、Ⅲ组患者VAS评分均明显降低（P〈0．05），Ⅱ组与Ⅲ组比较差异无统计学意义（P〉0．05）。【结论】预注100μg／kg氯胺酮或与异丙酚同时注射均能明显降低异丙酚注射痛的发生率和疼痛评分。[Objeetive]To compare the effect of different ways of infusing ketamine on propofol injection pain. [Methods] A total of 120 patients with ASA Ⅰ-Ⅱ selective surgery under endotracheal general anesthesia were randomly divided into three groups with 40 cases in each. Group Ⅰ and Ⅱ were injected with 2ml normal saline and 2mL 100μg / kg ketamine at lmin before the injection of propofol, and group Ⅲ was infused with the same dose of the mixture of ketamine and propofol. VRS and VAS scores were used to evaluate the effect for preventing propofol injection pain. [Results]Compared with the group Ⅰ , no pain scores of group U and Ⅲwere significantly higher （ P 〈0.05）, but there was no significant difference between group Ⅱ and Ⅲ （ P 〉0.05）. Compared with group Ⅰ , VAS scores of groupⅡ and Ⅲ were significantly lower （ P 〈0.05）, but there was no significant difference between groupⅡ and Ⅲ（ P 〈 0.05）. [Conclusion] Pre injection of 100μg/kg ketamine and/or injection of propofol at the same time can significantly reduce the incidence of propofol injection pain and pain score.
