摘 要:目的:探讨腰椎间盘突出症疼痛程度与矢状位、水平位及额状位CT区域定位的关系。方法:98例腰椎间盘突出症患者疼痛症状的严重程度按照WHO分级分为0~4度,突出类型按照胡有谷的影像学区域定位法,分析突出物的不同区域与疼痛的关系。结果:通过分析发现腰椎间盘突出症矢状位及水平位不同区域疼痛程度比较差异有统计学意义(P<0.05)。两两比较I、Ⅱ层面与III层面之间差异有统计学意义(P<0.05);3、4区与1、2区之间有统计学意义(P<0.05)。额状位不同区域之间比较差异无统计学意义(P>0.05)。结论:腰椎间盘突出症疼痛程度与突出物的影像学区域有关,矢状位I、Ⅱ层面及水平位3、4区为主的椎间盘突出疼痛程度相对较重。额状位分域与疼痛无明显关系。Objective:To study the relationship between the pain degree of lumbar intervertebral disc herniation(LIDH)and regional orientation by computer tomography(CT)from sagittal,horizontal and frontal planes.Methods:According to WHO standard,the pain in 98 patients with LIDH were classified from 0 to 4 grades,and the herniation type was classified according to Hu's imaging for regional orientation method.The relationship between the various regions of herniation and pain was analyzed.Results:The pain degree of various regions from sagittal plane and horizontal plane showed significant differences(P0.01).There was remarkable difference between layers of Ⅰ,Ⅱ and Ⅲ(P0.05)and also between Section 3/4 and Section 1/2(P0.05).As from frontal plane,there was no statistical difference between various regions(P0.05).Conclusion:The pain degree of LIDH is related to the herniation location oriented by imaging.When the herniation is located in Layer Ⅰ and Ⅱ in sagittal plane or in Section 3 and 4,the pain is likely to be heavier.Locations which are oriented from frontal plane have nothing to do with the degree of pain.
