高频彩超在诊断小儿肠旋转不良中的价值

摘　　要：目的探讨彩超在诊断小儿肠旋转不良中的价值。方法将该院收治并手术证实的21例肠旋转不良患儿的超声资料进行回顾性分析,并与其它影像学检查相对比。结果17例术前彩超观察到SMV和SMA的位置关系异常:5例SMV位于SMA前方,12例SMV位于SMA左侧,17例二维超声均显示有中低回声光团,呈"螺旋征"或"靶环征",彩色多普勒显示为红蓝相间的环绕血流,频谱多普勒可测得动脉、静脉频谱,明确诊断本病;4例超声检查阴性者有2例于术后随访观察发现1例位置关系正常,1例SMV位于SMA左侧,另外2例失访。结论超声检查安全可靠、简便易行、快捷准确、无放射性、且不需要特殊造影剂,为筛查本病的一种好方法。Aim To evaluate the value of diagnosis with Doppler ultrasound in patients with malrotation of intestine. Methods 21 cases proved to be malrotation of intestine with surgical operation have been retrospectively reviewed and compared with other images. Results The interrelationship of SMV and SMA in 17 cases observed before operation were abnormal ： SMV of 5 cases were in front of SMA, 12 cases were to the left. There were middle and low anechoic follicles in 17 cases with two-dimensional uhrasonography. ＂Spiral sign＂ or ＂target sign＂ was shown. Red and blue circular blood streams were shown in Color Doppler uhrasonography. Doppler ultrasonography can measure the arteries and veins to confirm this disease. In the 2 of the 4 negative cases that were observed after operation, one proved to have the normal interrelationship, the other proved to have SMV to the left of SMA. The interrelationship of the other 2 cases of the 4 with ultrasonography were lost. Coulusions Color doppler uhrasono-graphy , flexibility, convenience, no-trauma, no-radioactivity, was good idea for malrotation of intestine.
