国产封堵器介入治疗先天性心脏病20例临床分析

摘　　要：目的应用国产封堵器介入治疗先天性心脏病并对其疗效进行初步评估。方法总结我院2005年7月-2009年5月共有20例先天性心脏病患者介入治疗。其中室间隔缺损（VSD）（膜部）8例，房间隔缺损（ASD）（继发孔型）6例，动脉导管未闭（PDA）6例。结果20例先天性心脏病患者中一例室间隔缺损行左心室造影示缺损小于3mm，终止介入治疗；一例室间隔缺损行左心室造影示两个小缺损，面积共4mm，后转心胸外科行室缺修补术；余18例皆获得满意的临床效果，成功率为90％。有一例室间隔缺损介入治疗术后第五天出现完全性房室传导阻滞（CAVB），立即行临时心脏起搏器安置术，地塞米松10mgIV3天后CAVB恢复正常，其他患者无不良并发症发生。随访2月～4年，无不良并发症。结论国产封堵器用于先天性心脏病介入治疗安全有效，价格低廉，值得临床推广使用。Aim To assess preliminary efficacy of domestic occluder interventional therapy for congenital heart disease. Methods From July 2005 to May 2009, there were a total of 20 cases of interventional therapy with congenital heart disease in our hospital, including 8 cases of ventricular septal defect （VSD） （ membranous ） ,6 cases of atrial septal defect （ASD） （ secondary pass ） ,6 cases of patent ductus arteriosus （PDA）. Results Among these 20 cases,interventronal treatment stoppd in 1 case of ventricular septal defect with left ventricular angiography showing less than 3 mm 1 case of ventricular septal with left ventricular angiography defect line showing two small defects,a total area of 4mm,was transferred and performed cardiothoracic surgery for VSD repair. The rest 18 cases all had satisfactory clinical results ,with a success rate of 90%. After the fifth day of interventional therapy one case of VSD showed complete atrioventricular block （CAVB）, then an immediate temporary pacemaker implant was conducted and applied with dexamethasone 10 mg IV before recovery with normal CAVB 3 days later. The other cases with no adverse disease occurred, and all were followed up for 2 months to 4 years with no adverse complications. Conclusion Domestic occluder interventional therapy for congenital heart disease is safe and effective, inexpensive and worthy of promoting for clinical use.
