Suresight视力筛查仪筛查儿童弱视界值的评价

摘　　要：【目的】对前期应用Suresight视力筛查仪检测儿童屈光度确定的不同界值[1]进行弱视筛查意义的评价。【方法】对190例患儿,380只眼进行Suresight筛查屈光度和散瞳检影验光检查,选择前期研究[1]中Suresight视力筛查仪检测所得的屈光度不同界值,计算不同弱视筛查阳性界值的灵敏度、特异度和阳性预测值,评价其对弱视筛查的意义。【结果】前期研究提出的以球镜值偏离P25～P75,柱镜绝对值偏离P75作为弱视高危人群的阳性界值筛查弱视的灵敏度较高,分别为78%和86.1%;而特异度较低,分别为69%和49%;阳性预测值分别为78.4%和70.9%。以球镜值偏离P5～P95,柱镜绝对值偏离P95为作为可疑人群的阳性界值筛查弱视的特异度和阳性预测值较高,特异度分别为91.3%和90.3%,阳性预测值分别为87.4%和89.2%。【结论】依据Suresight筛查结果不同界值制定的弱视高危和可疑儿童管理标准简单易行,值得向基层推广。对弱视高危儿童进行随访,对可疑儿童及时转诊,提高弱视防治率。[Objective] To evaluate the cutoff points of suresight vision screening which were acquired form preliminary study for children with amblyopia. [Methods] 190 cases of children, 380 eyes were performed with Suresight screening and cycloplegia, confirmed amblyopic and non-amblyopic. The pre-study screening cutoff points were chosen to calculate their sensitivity, specificity and positive predictive value for the evaluation of amblyopia screening respectively. [Results] When spherical len deviated from P25-P75 or cylindrical len exceeded P25, it had higher sensitivity of 78%, 86.1% ; lower specificity of 69 %, 49 % ; positive predictive value of 78.4%, 70.9 %, which was considered as high-risk of amblyopia children of before. When spherical len deviated from P5-P95 or cylindrical len exceeded P95 , it has higher specificity of 91.3% ,90. 3% and higher positive predictive value of 87.4% .89.2%, which was considered as suspicious children before. [Conclusions] The high-risk and suspicious criterion based on the different cutoff points of Suresight screening is simple and easy for the grass-roots promotion. It requires close follow-up in high-risk children and timely referral of suspicious children to improve prevention and treatment of amblyopia efficiency.
