飞行员腹腔镜胆囊切除术与开腹胆囊切除术的疗效对照研究

摘　　要：目的探讨腹腔镜胆囊切除术与开腹胆囊切除术对空勤人员的术后放飞行及飞行状况的影响。方法对10例飞行员腹腔镜胆囊切除术(LC组)与同期8例飞行员开腹胆囊切除术(OC组)的临床治疗或术后飞行情况随访观察资料,对两组人员的手术适应证、术式选择、术中出血量、机体组织创伤、手术时间、术后下床活动时间、胃肠功能恢复、进食时间、术后的地面观察、放飞时间、飞行状况等进行对比分析。结果两组患者术中、术后均无并发症发生,全部痊愈。而在临床治疗、术后恢复及术后放飞时间等方面LC组均优于OC组。结论LC组同OC组相比较明显具有创伤小、痛苦轻、腹腔脏器受干扰少、全身应激反应轻、术后恢复快和放飞时间早等优点。Objective To study postoperative flying and flying condition effects of pilots who received laparoscopic cholecystectomy or open cholecystectomy. Methods During 1991 to 2006, ten pilots underwent laparoscopic cholecystectomy （ LC group） and eight pilots underwent open cholecystectomy （ OC group ）. Analysis of operative indications, operative modes, bleeding volume during the operations, damage of body tissue, duration of the operations, time for recovery of activities, restoration of food intake, ground observation, time of re-flying, flying conditions and so on. Results LC group and OC group didn＇t show any complications during and after operation. They all convalesced completely. But LC g^oup surpassed OC group in clinical care, postoperative restoration and time of re-flying. Conclusions Comparison of LC group with OC group showed that LC has the advantages of less damage, lighter pain, less disturbance for ab- dominal organs, lighter stress of the body, quicker recovery and early re-flying.
