尖角T波的临床意义

摘　　要：目的研究尖角T波的临床意义。方法对本院近年来住院病例中发现的23例心电图上出现的尖角T波进行分析。结果V1导联2例,V2导联19例,V3导联1例,V2、V3导联同时出现1例;尖角T波时限平均7.22±1.35m s;尖角T波振幅平均0.38±0.14mV。尖角T波患者中,室间隔缺损11例、房间隔缺损8例、房间隔缺损合并室间隔缺损2例、室间隔缺损合并右室双出口1例、3例合并有肺动脉高压,另有1例缺乏超声资料,临床诊断不详。13例患儿行房间隔缺损和室间隔缺损修补术,术后1周复查心电图,尖角T波消失者12例,减小者1例。结论尖角型T波常见于先心病室间隔缺损、房间隔缺损的患儿。手术修补缺损或封堵术后,圆顶尖角型T波可以减小或消失。Objective To investigate the clinical significance of dart T wave. Methods On an analysis of 23 patients whose electrocardiogram appeared Pointed T-wave. Results 17 cases of male,6 cases of women,7.35 ± 6.29 year-old age.Leads of dart T wave： 2 cases appeared on lead V1;19 cases,V2 lead;lead 1 case,V3;V2 and 1 case,V3.Dart T wave duration： 7.22 ± 1.35ms,dart T wave amplitude： 0.38 ± 0.14 mV.Patients with dart T wave,11 cases of ventricular septal defect,8 cases of atrial septal defect,2 cases of atrial septal defect combined ventricular septal defect,1 case of ventricular septal defect combined double-outlet right ventricle,3 cases of pulmonary hypertension,another case,lack of clinical data.13 cases of patients with atrial septal defect and ventricular septal defect underwent operation,the dart T wave disappeared in 12 cases one week after operation. Conclusion Dart T wave is commonly seen in congenital heart disease with ventricular septal defect and atrial septal defect.After surgical repair or transcatheter closure,dart T wave can reduce or disappear.
