摘 要:目的探讨袖状胃切除术的减肥作用及机制。方法通过高脂饮食诱导SD肥胖大鼠,然后随机分为实验组和对照组。实验组施以袖状胃切除术,对照组施以假手术。术后连续观察8周,比较两组术后体质量、摄食和外周血ghrelin,GLP-1,PYY3-36浓度的变化。结果实验组术后摄食量明显下降(P<0.0 5),体质量连续3周持续下降,然后缓慢回升,至第8周时体质量仍低于术前,外周血ghrelin浓度下降,GLP-1和PYY3-36浓度升高(P<0.0 5),随着时间延长,ghrelin浓度逐渐升高,血GLP-1和PYY3-36浓度逐渐回落。对照组术后食量、血ghrelin、GLP-1和PYY3-36浓度无明显变化(P>0.0 5),8周后体质量明显高于术前(P<0.0 5)。结论肥胖大鼠袖状胃切除术减重效果确切而持久,其术后ghrelin浓度下降,GLP-1及PYY3-36浓度升高致使摄食量减少是其减肥的主要作用机制。Objective To investigate the weight loss mechanism of sleeve gastrectomy.Methods SD rats were fed with high-fat diet to induce obesity,and then the obesity rats were randomly divided into experimental group(underwent SG) and control group(underwent sham surgery).Eight weeks after operation,the body weight,food intake and peripheral blood concentration of Ghrelin,GLP-1,PYY3-36 of rats were checked.Results Compared with controll group,in experimental SG model,food intake was decreased(P0.05);the postoperative weight continued to decline for three weeks and then slowly recovered,and body weight was still lower than the preoperative 8 weeks later,while Ghrelin decreased and GLP-1,PYY3-36 increased(P0.05).With time,Ghrelin gradually increased and GLP-1,PYY3-36 gradually declined.In the control group,compared with peroperation,the postoperative weight markedly increased except one week after operation,and 8 weeks later,weight was significantly higher than preoperation;but food intake,Ghrelin,GLP-1 and PYY3-36 had no significant change(P 0.05).Conclusions The weight loss of sleeve gastrectomy in DIO rats's decline is lasting and obvious.The postoperative effect of decreased Ghrelin and increased GLP-1,PYY3-36 is the main mechanism for weight loss.
