老年人急性脓胸的电视胸腔镜手术治疗

摘　　要：目的探讨老年人急性脓胸的电视胸腔镜手术治疗效果。方法38例老年急性脓胸患者,均采用全电视胸腔镜手术治疗,术中充分清除脓液及坏死物质,充分冲洗并促肺膨胀,放置引流。结果38例患者全部治愈出院,手术时间20～75分钟,平均42分钟,平均术后住院10天,无手术死亡及严重并发症,随访无复发病例。结论电视胸腔镜下脓胸清除术是治疗老年人急性脓胸的有效手段,尽早手术是取得手术成功的关键。Objective To evaluate the effect of acute empyema treated by video-assisted thoracoscopic surgery （VATS） in the elderly. Methods A total of 38 patients were performed VATS. In the course of operation, pus, and necrotic tissue were cleared, the compressed lung was fully stripped after thoracic cavity were fully cleaned, and at last drainage tube was put. Results All of the 38 eases recovered well with no operative death and severe complications. The operation time ranged between 20 and 75 minutes, with an average of 42 minutes. The mean postoperative hospital stays were 10 days. No recurrence of empyema was observed at the follow-up examination. Conclusion VATS is effective and safe in the elderly with acute empyema. Early surgery is the key to success.
