无张力疝修补术76例体会

摘　　要：目的总结无张力疝修补术的临床经验和体会。方法总结分析76例无张力疝修补术的临床资料和手术方法。结果本组病人全部治愈,无术后复发,有1例疼痛经保守治疗好转;1例肥胖嵌顿性斜疝患者,术后术野及阴囊肿胀,经乙醇湿敷消退。结论要求术者熟练掌握Bassin、iFergerson、Halsted等经典疝修补手术的操作技术,清晰腹股沟的解剖结构,术中能辨认神经、精索结构和精索,精确安置补片的位置,使用质地适中补片,以降低复发率,减少并发症。老年疝或复发疝宜尽量选择无张力疝修补术。Objective To summarize the clinical experience of tension - free hemioplasty in 76 cases. Methods Clinical data and operative procedures of 76 patients with tension - free hemioplasty since 2004 were retrospectively analyzed. Results All these cases were cured with no recurrence, but there was postoperative swelling in surgical field and scrotum occurred in one fat case of incarcerated hernia,and it was subsided after alcohol wet packing. Conclusion The operators should master the classic surgical procedures such as those of Bassini, Fergerson, Halsted etc and clearly realize the anatomical structure of inguinal region, and they can identify nerve, spermatic cord structure and spermatic cord during the operation. The patch should be put in a proper position. In order to decrease the recurrence rate and reduce complications in old men with hernia or recurrent hernia, it should be operated with tension - free hemioplaty as far as possible.
