摘 要:目的探讨本院2007~2008年医院真菌感染病原菌分布特点、耐药特征及其变迁情况,指导临床合理应用抗真菌药物。方法采用改良沙堡培养基、科玛嘉念珠菌显色培养基及API220CAUX酵母菌鉴定培养基培养及鉴定真菌,采用法国梅里埃公司ATBFungus3药敏卡进行药敏试验。结果医院真菌感染病原菌分布以白念珠菌为主,但其构成比2年间下降了10.34%(P<0.001),而热带念珠菌及克柔念珠菌的构成比分别增加了5.71%,2.42%(P<0.05),2年间真菌对氟康唑、伊曲康唑的耐药率提高了4.92%,5.25%,经统计学检验差异有统计学意义(P<0.05)。结论近2年来本院真菌感染病原菌分布发生了变化,对唑类药物的耐药情况日趋严重,应加强对临床真菌感染与耐药性情况的监测,以指导临床合理使用抗生素。Objective To analyze the drug-resistant and pathogenic fungus distribution in nosocomial fungal infections from 2007 to 2008 in our hospital, and to guide reasonable using of antifungal agents in clinic. Methods The fungi were cultured by Saboraud medium,CHROMagar medium and API220C AUX medium, drug susceptibility tests were conducted by using ATB-Fungus3. Results Candida albicans remained the main pathological fungus in clinic although its constituent ratio decreased 10.34% ( P 〈 0. 001 ), while the constituent ratio of Candida tropicalis and Candida krusei increased 5.71% and 2.42% (P 〈 0.05 )during the past two years. The drug-resistant rate of fungus to fluconazoleand itraconazole increased 4.92% and 5.25% ( P 〈 0.05 ) during the past two years. Conclusions In the past :2 years,the distribution of nosocomial fungal infections in our hospital had changed in the hospital, and the drug-resistance to azolcswas increased seriously. We should pay more attention to the monitor of clinical fungus infection and its drugresistance in order to guide the reasonable use of antibiotics in clini.
