体态语言机的设制及在机械通气患者护理中的应用

摘　　要：目的评价自行设制的体态语言机(下称语言机)在气管插管行机械通气致暂时性失语患者护理中的应用价值。方法调查100例气管插管致暂时性失语患者及6名护理专家审核确认患者12项需求,根据需求项目制作相应的体态语言图像符号,利用嵌入式微处理器技术,联合电子显示屏幕制作出语言机。将150例行气管插管行机械通气患者随机分为三组各50例,写字板组由患者在写字板上写明需求,图片组由护理人员根据患者表示出示图片由患者指认需求,语言机组由患者或陪护按语言机界面提示指令需求。结果三组患者在气管插管行机械通气期间共产生了1014次需求指令,遵医率均为100%;明确12项需求所需总时间语言机组显著短于写字板组、图片组[(19.61±12.22)s、(143.25±96.74)s、(127.35±117.46)s,均P<0.01];语言机组明确12项需求所需时间显著短于其他两组(均P<0.01)。结论应用语言机可快捷准确地明确气管插管行机械通气暂时性失语患者的各项需求,实现更高的护患交流效率。Objective To evaluate the effect of a homemade body language machine used in patients with temporary aphasia resulting from mechanical ventilation. Methods Twelve items of patients＇ needs were verified by investigating 100 mechanically ventilated patients who suffered from temporary aphasia, and by consulting 6 nursing experts. Then body-language machine was made in com- bination of electronic display with body-language images which were designed according to the need items and embedded micropro- cessor technique. A total of 150 mechanically ventilated patients were randomized into 3 groups equally. Patients in group 1 wrote down their needs on a writing-board, patients in group 2 used pictures provided by nurses to express their needs, and those in group 3 used the body-language machine. Results All patients expressed needs for 1014 times, and all of them showed good comp- liance. The total time for identification of 12 items of patients＇ needs in group 3 was（19. 61 ±12.22）s, and it was significantly shorter than those in group 1 and group 2, with the time being （143.25±96.74）s,（127.35±117.46）s respectively. The time for identification of 12 items of patients＇ needs in group 3 was significantly shorter than those in the other two groups （P〈0.01 for all）. Conclusion Application of body-language machine can understand patients＇ needs quickly and accurately and improve effectiveness of nurse-patients communication.
