摘 要:目的观察食物过敏症婴幼儿血清中IL-10和转化生长因子-β(TGF-β)的变化,探讨其在婴幼儿食物过敏症致病中的作用。方法选择食物过敏症婴幼儿40例(食物过敏组),另选取年龄、性别相匹配的健康婴幼儿40例作为健康对照组,通过Uni-CAP100E系统检测食物过敏组及健康对照组婴幼儿血清食物特异性IgE(SIgE),采用ELISA法检测2组婴幼儿血清IL-10和TGF-β水平。结果食物过敏组婴幼儿均有皮肤、呼吸道和(或)消化道表现,血清食物SIgE检测有一种或多种食物SIgE阳性。其中牛奶过敏34例,鸡蛋过敏27例,且SIgE定量分析均在2级以上。健康对照组婴幼儿血清食物SIgE阴性,且平时无食物过敏的临床表现。食物过敏组血清IL-10水平[(10.28±2.38)ng.L-1]较健康对照组[(4.75±0.92)ng.L-1]显著升高(P=0.035)。食物过敏组TGF-β水平[(1 041.93±75.64)ng.L-1]显著高于健康对照组[(734.29±39.67)ng.L-1](P=0.001)。结论食物过敏是由免疫机制介导的过敏性疾病,细胞因子IL-10和TGF-β可能参与了婴幼儿食物过敏的发生,并可能在其中发挥重要作用。Objective To investigate the role of interleukin - 10 (IL - 10) and transforming growth factor bata (TGF -β) in the pathogenesis of food allergy in infants. Methods The serum food specific immunoglobulin E (SIgE) were detected in all infants with food allergy (food allergy group,n =40) and the normal control group( n = 40) by Pharmacia UniCAP100E system. The serum levels of IL- 10 and TGF-β in food allergy group and normal control group were detected with enzyme linked immunosorbent assay(ELISA). Results Forty infants with food allergy had signs in the skin,the respiratory and (or) the digestive tract, and they had one or several food SIgE positive in their serum food SIgE detection, including eggs allergy in 27 cases, milk allergy in 34 cases, and quantitative analysis of SIgE in them was all above grade Ⅱ. The serum food SIgE was negative in the controls, and they had no clinical manifestations of food allergy. There was significant difference between serum level of IL - 10 in the infants with food allergy [ ( 10.28 ±2.38) ng ·L^-1] and in the normal control group [ (4.75 ± 0.92) ng·L^-1 ] ( p = 0. 035 ). The serum level of TGF - β in the infants with food allergy [ ( 1 041.93 ± 75.64 ) ng ·L^-1 ] was higher significantly than that in normal control group [ (734.29 ± 39.67 ) ng·L^-1] ( P = 0. 001 ). Conclusions Food allergy is disturbance of the immune system,IL- 10 and TGF - β may play a key role in the pathogenesis of food allergy in infants.
