颈动脉粥样硬化与冠心病关系预测价值

摘　　要：目的利用体表高频超声检查研究颈动脉粥样硬化与冠状动脉粥样硬化关系的预测价值。方法对283例冠状动脉造影受检者行颈总动脉超声检查，测量其内中膜厚度、斑块厚度、计算斑块积分。并将其结果与冠状动脉造影及心血管危险因素进行单因素的t、χ^2检验。结果①颈动脉粥样硬化斑块、内中膜厚度（IMT）及斑块＋IMT对冠心病的阳性预测值分别为84．67％、81．39％及87．78％，三者对冠心病的特异性分别为71．60％、90．12％及95．06％；②冠状动脉造影阳性组的颈动脉内中膜厚度和粥样硬化斑块发生率明显高于阴性组，且随病变支数的增加，颈动脉内中膜厚度和斑块积分均增加（P〈0．05）；⑧冠心病组年龄、男性、高密度脂蛋白、血糖、血压水平方面显著高于对照组。结论颈动脉粥样硬化斑块、IMT可作为冠心病预测因子，两者联合应用，对冠心病的预测可提供更多信息。Objective To evaluate the relationship between carotid atherosclerosis and coronary heart disease（CHD）. Methods 283 examinees who underwen coronary angiography（CAG） for suspected CHD were investigated by ultrasound imaging of the common carotid for measure ment of intimia-media thickness（IMT） and crouse＇ s score, of plaqur, one-sample T test and chi-square test were carried out between the results of ultrasound examination and CAG. Results ①The positive preditire value of carotid plaque and IMT for CHD was 84. 67% and 81.39% respectively, Preclictive value of both carotid plaque and IMT was 87.78% , the specificity of the three group was 71.60% ,90. 32% and 95.06% respectively. ②The IMT and incidence of common carotid plaque in pationts with positive CAG lvere significantly higher than the subjects with negative CAG, a linear correlation between IMT, crouse＇ s score and the extent of CHD was found （ P 〈 0. 05 ）. ③The age, male gender, HDL-C ,Glu and blood pressure in patients with positive CHD were significantly higher than the subjects with negative. Conclusion The preclictor of the CHD with caroticl plaque and IMT, they bewteen joint apply and furnish to more information in preclicting CHD.
