摘  要:目的了解肺炎克雷伯菌(KPN)老年患者分离株抗菌药物耐药状况和膜孔蛋白基因分子进化状况。方法对38株分离自老年患者的KPN进行了20种抗菌药物敏感性检测和膜孔蛋白基因(ompK36)测序及分子进化分析。结果38株分离自老年患者的KPN对抗菌药物敏感性低,38株均检出ompK36基因,产物经测序比对,各株分别获得623656bp同源序列;敏感株(2号株)为650bp,8、9、11、23号株获得656bp,为CGGCGA小片段插入;其余各株分别小片段缺失。结论38株分离自老年患者的KPN连续分离株对20种抗菌药物敏感性低,KPN连续分离株膜孔蛋白ompK36基因具有较好的多态性。OBJECTIVE To investigate the status of in vitro susceptibility to antimicrobial agents and polygenetic analysis of porin genes of Klebsiella pneumoniae (KPN) isolates from elderly patients. METHODS Thirty-eight successively isolated strains of KPN were collected from elderly patients, and in vitro susceptibility to 20 antimicrobial agents was detected, then DNA sequencing and polygenetic analysis of porin genes (ompK36) were processed. RESULTS Thirty-eight strains of KPN isolates from elderly patients showed high resistance to antimicrobial agents, and porin genes (ompK36) were all detected in these 38 strains. Multiple sequence aignment showed that sequence homology(623-656 bp) was obtained by every strain, strain No. 2 (showed sensitivity to antimicrobial agents) obtained 650bp sequence homology; and strain No. 8, 9, 11 and 23 obtained 656bp sequence homology, that was small insertion CGGCGA. However, the rest showed small deficiency. CONCLUSIONS Thirty-eight strains of KPN isolates from elderly patients show high resistance to antimicrobial agents, and polygenetic analysis of porin genes (ompK36) show good genetic polymorphism.
